摘要
目的研究血清恶性肿瘤特异性生长因子(TSGF)、甲胎蛋白(AFP)、血管内皮细胞生长因子(VEGF)水平变化与肝癌患者TNM分期的相关性。方法选取解放军第91中心医院2015年6月-2017年11月收治的126例肝脏疾病患者作为观察组,其中肝癌患者62例,良性肝脏疾病者64例,另选同期健康体检者65例作为对照组,比较两组血清TSGF、AFP、VEGF水平、不同TNM分期肝癌患者血清TSGF、AFP、VEGF水平,并分析血清TSGF、AFP、VEGF水平与肝癌患者TNM分期相关性。结果单因素方差分析,观察组血清TSGF、AFP、VEGF水平与对照组比较,差异有统计学意义(P<0. 05);肝癌患者血清TSGF、AFP、VEGF水平高于良性肝脏疾病者、健康体检者(P<0. 05);肝癌患者血清TSGF、AFP、VEGF水平随TNM分期增高呈逐渐上升趋势(P<0. 05);经Spearman相关分析可知,血清TSGF、AFP、VEGF水平与肝癌患者TNM分期呈正相关(r1=0. 731,P1=0. 007; r2=0. 621,P2=0. 005; r3=0. 472,P3=0. 014)。结论血清TSGF、AFP、VEGF在肝癌患者中呈现异常升高状态,且随TNM分期增高呈逐渐上升趋势,与TNM分期呈正相关,有助于评估病情程度,为临床治疗及病情监测提供信息支持。
Objective To explore the correlation between the changes of serum malignant tumor specific growth factor（ TSGF）,alpha fetoprotein（ AFP）,vascular endothelial growth factor（ VEGF） and TNM staging in patients with liver cancer.Methods 126 cases of liver diseases in our hospital from June 2015 to November 2017 were selected as the observation group（ 62 cases of liver cancer and 64 cases of benign liver disease）.65 healthy persons in the same period were taken as the control group.The serum levels of TSGF,AFP,VEGF and serum TSGF,AFP and VEGF in the patients with liver cancer from different TNM stages were compared.The correlation between serum levels of TSGF,AFP and VEGF and TNM staging in patients with liver cancer was also analyzed.Results The serum levels of TSGF,AFP and VEGF in the observation group were significantly different from those in the control group by single factor analysis of variance（ P0.05）.The serum levels of TSGF,AFP and VEGF in patients with liver cancer were higher than those in benign liver diseases and healthy persons（ P0. 05）.The levels of serum TSGF,AFP and VEGF were gradually increased with the increase of TNM stage（ P〈0. 05）. The spearman correlation analysis showed that serum TSGF,AFP and VEGF levels were positively correlated with TNM stage in patients with liver cancer（ r1= 0.731,P1= 0.007; r2= 0. 621,P2= 0. 005; r3= 0.472,P3= 0.014）. Conclusion The serum TSGF,AFP and VEGF showed an abnormal rising state in the patients with liver cancer,and were gradually increased with the increase of TNM staging. It had a positive correlation with the TNM staging.It is helpful to evaluate the degree of diseases and provide information support for clinical treatment and monitoring of diseases.
