摘要
[目的]探讨中医手法复位结合经皮中空双加压螺钉微创治疗锁骨中段骨折的手术方法和治疗效果。[方法]我院自2010年9月至2017年12月,运用中医正骨钳持端提回旋手法复位结合经皮中空双加压螺钉微创治疗38例锁骨中段骨折患者。其中男17例,女21例;年龄18~62岁,平均年龄35岁。观察患者手术时间、术中出血量、住院时间、复位情况,参考Neer肩关节功能评定标准评定优良率。[结果]本组患者手术时间20~45min,平均30.2min。术中出血量约10~20mL。住院日4~8d,平均5.2d。38例患者均获随访,随访时间6~26个月,平均10.3个月。37例患者8~12周达临床愈合,平均愈合时间9.6周,其余1例延时至20周愈合,愈合率达100%。肩关节功能评定,优34例、良4例、可0例、差0例,优良率100%。[结论]应用中医正骨钳持端提回旋手法复位结合经皮中空双加压螺钉微创治疗锁骨中段骨折,是一种治疗有移位的锁骨中段骨折的有效方法,值得临床推广。
[Objective] To explore the operative method and therapeutic effect of minimally invasive treatment of middle clavicular fracture with manual reduction and pereutaneous hollow double compression screw. [Methods] From September 2010 to December 2017, thirty eight cases of middle clavicular fracture were treated with traditional Chinese medicine orthopedic forceps holding back manipulation and percutaneous hollow double compression screw. There were 17 males and 21 females, aged 18-62 years, with an average age of 35 years. Operating time, bleeding volume, length of stay, Joint reduction and shoulder joint function were observed after treatment. [Results] Operating time of this group was 20～45min, with an average of 30.2min. The amount of bleeding was about 10～20mL during the operation. The hospital day was 4-8d, with an average of 5.2d. All 38 patients were followed up for 6-26 months, with an average of 10.3 months. After 8-12 weeks, 37 patients reached clinical healing, the average healing time was 9.6 weeks, and another 1 case was healed up to 20 weeks, and the healing rate was 100%. According to the Neer shoulder joint function evaluation criteria, 34 cases were excellent, 4 good, 0 fair, and 0 poor, with an excellent and good rate of 100%. [Conclusion] It is an effective method to treat the displacement middle clavicular fracture with minimally invasive treatment, and it is worth further promoting in clinic.
