克氏针辅助内侧解剖加压钢板治疗胫骨远端骨折的临床疗效

摘要 目的探讨克氏针辅助内侧解剖加压钢板治疗胫骨远端骨折的临床效果。方法对33例胫骨远端骨折患者实施克氏针辅助内侧解剖加压钢板治疗,置入后,定期行X线检查随访确定骨折愈合情况;钢板拆除后,按照Joher-Wruhs评分系统评估治疗效果。结果 33例患者中32例长期随访,1例6个月后失访。随访患者切口均愈合(均为I期愈合),愈合时间为11~20周,平均愈合时间(14.36±2.31)周;骨折复位均为优良(移位<5~10 mm),无力线不良、骨不连、延迟愈合、畸形愈合等不良情况出现。随访患者术后均未出现钢板松动、断裂及螺钉断裂情况。治疗优良率为96.88%。结论采用克氏针辅助内侧解剖加压钢板治疗胫骨远端骨折,效果确切,患者骨折复位良好,无不良愈合状况出现,值得临床推广应用。 Objective To investigate the clinical effect of Kirschner wire assisted medial anatomical compressionplate in the treatment of distal tibial fractures. Methods A total of 33 patients with distal tibial fractures were treatedwith Kirschner wire assisted medical anatomic compression plate. After implantation, X-ray examination was performedregularly to determine fracture healing. After removal of the plate, the therapeutic effect was evaluated according toJoher-Wruhs scoring system. Results Of the 33 patients, 32 patients were followed up for a long time, and 1 patientwas lost after 6 months. The incisions of patients followed up healed (1 stage healing) in 11 -20 weeks, and theaverage healing time was (14.36依2.31) weeks. Patients had excellent fracture reduction (displacement〈5-10 mm). Nobad alignment, nonunion, delayed union, malunion and other adverse situations occurred. No plate loosening, breakageand screw breakage occurred after surgery in patients followed up. The excellent and good rate of treatment was96.88%. Conclusion Kirschner wire assisted medial anatomical compression plate in the treatment of distal tibialfractures has a definite curative effect and the fracture reduction is good, no adverse healing. It is worthy of clinicalpopularization and application.
