摘要
目的探讨2型糖尿病肾病(T2DN)患者血清网膜素(omentin)-1水平及其临床意义。方法将125例T2DN患者作为糖尿病肾病(DN)组,125例无肾脏损害的2型糖尿病患者作为糖尿病组,125例健康体检者作为对照组。比较3组研究对象的omentin-1及各项生化指标水平,分析DN组患者omentin-1水平的影响因素。结果 DN组和糖尿病组的糖化血红蛋白(Hb A1c)、胰岛素抵抗(HOMA-IR)指数、C反应蛋白(CRP)、FBG和TG水平均高于对照组,LDL-C和omentin-1水平均低于对照组(均P<0.05);DN组血清肌酐、CRP和尿素氮水平高于糖尿病组,空腹胰岛素、omentin-1、FBG、Hb A1c和TG水平低于糖尿病组(均P<0.05)。DN组患者血清omentin-1水平与估算肾小球滤过率(e GFR)水平呈正相关,与CRP水平呈负相关(P<0.05);糖尿病组患者omentin-1水平与HDL-C水平呈正相关,与HOMA-IR、CRP水平呈负相关(P<0.05)。e GFR是DN组患者血清omentin-1水平的独立影响因素(P<0.05)。结论 T2DN患者血清omentin-1水平降低或可用于评估患者肾功能的损害程度。
Objective To investigate the serum omentin-1 level and its clinical significance in the patients with type 2 diabetic nephropathy( T2 DN). Methods A total of 125 patients with T2DN were enrolled as diabetic nephropathy( DN) group,125 patients with type 2 diabetes and without kidney damage as diabetic group,and 125 healthy people undergoing check-up as control group. The levels of omentin-1 and biochemical indicators were compared among the three groups,and the influencing factors of the omentin-1 level were analyzed in the DN group. Results In the DN group and diabetic group,the levels of hemoglobin A1c( HbA1c),Homeostasis Model of Assessment for Insulin Resistance( HOMA-IR) index,C-reactive protein( CRP),FBG and TG were higher,and the levels of LDL-C and omentin-1 were lower than those in the control group( all P〈0. 05); the DN group had higher levels of serum creatinine,CRP and blood urea nitrogen,and lower levels of fasting insulin,omentin-1,FBG,HbA1c and TG compared to the diabetic group( all P〈0. 05). In the DN group,the serum omentin-1 level positively correlated with the level of estimated glomerular filtration rate( e GFR),and negatively correlated with the CRP level( P〈0. 05). In the diabetic group,the omentin-1 level positively correlated with the HDL-C level,and negative correlated with the HOMA-IR and CRP levels( P〈0. 05). e GFR was an independent influencing factor of serum omentin-1 level of the patients in the DN group( P〈0. 05). Conclusion The decrease of serum omentin-1 level might be used to assessing the severity of kidney function damage in the patients with T2DN.
