摘要
目的:观察佛手定痛汤结合针灸、耳穴敷贴法治疗紧张型头痛的效果。方法:将我院自2015年1月~2017年8月收治的紧张型头痛患者纳入研究范围,将其就诊编号按随机分层法分为观察组(n=47)与对照组(n=50),对照组给予针灸、耳穴贴敷,观察组则在此基础联合佛手定痛汤,对比两组治疗前后主症、次症、兼症等症状的中医症候积分、头痛发作次数、持续时间及急性止痛药物服用天数,并于治疗前后进行头痛影响实验(HIT)及症状自评(SCL-90)量表、生活质量(SF-36)量表评分,对比OAB患者头痛影响程度及心理状态、生活质量。结果:治疗后,(1)两组主症、次症、兼症等症状的中医症候积分及头痛次数、头痛持续时间、急性止痛药物使用天数均显著下降,且观察组低于对照组;(2)两组HIT实验影响力等级均显著改善,且观察组改善程度优于对照组;(3)两组SCL-90量表各维度分值均显著下降,且观察组低于对照组;(4)两组SF-36量表各维度分值均显著上升,且观察组高于对照组,上述对比差异均有统计学意义(P<0.05)。结论:佛手定痛汤结合针灸、耳穴敷贴法治疗紧张型头痛能获取更佳的症状缓解获益,可有效改善患者心理状态及生活质量。
Objective：To explore the effect of Foshou Dingtong Decoction combined with acupuncture and auricular acupoint application for tension-type headache（TTH）.Methods：Patients with TTH who were admitted to the hospital from January2015 to August 2017 were enrolled in the study.They were randomly divided into observation group（47 cases） and controlled group（50 cases）.The controlled group was treated with acupuncture and auricular acupoint application.On the basis,the observation group was additionally treated with Foshou Dingtong Decoction.The TCM symptom scores,attacks and duration of headache and time of using painkillers were compared between the two groups before and after treatment.The severity of headache,mental status and quality of life were evaluated with the headache impact test（HIT）,symptom check-list（SCL-90） and quality of life scale（SF-36）.Results：After treatment,（1）The TCM symptom scores,attacks and duration of headache and the time of using painkillers in the two groups were decreased/shortened significantly,showing the observation grou Pthe controlled group.（2）The grade of effect of HIT was improved significantly,and the improvement in the observation group was better than that in the controlled group.（3）Scores of different dimensions of SCL-90 in the two groups were decreased significantly,showing the observation grou Pthe controlled group.（4）Scores of different dimensions of SF-36 in the two groups were increased significantly,showing the observation grou Pthe controlled group（P〈0.05）.Conclusion：Foshou Dingtong Decoction combined with acupuncture and auricular acupoint application can well relieve symptoms and effectively improve the mental status and quality of life of patients with TTH.
