摘要
目的探讨慢性紧张型头痛伴抑郁症的中医治疗及临床预后。方法将我院2018年6月至2019年7月慢性紧张型头痛伴抑郁症患者80例,随机分组,常规西药盐酸乙哌立松片治疗组就诊患者采取常规西药盐酸乙哌立松片治疗,自拟中药汤剂治疗组对本次就诊患者采取自拟中药汤剂治疗。比较两组慢性紧张型头痛伴抑郁症疗效;慢性紧张型头痛缓解的时间、抑郁症状缓解时间;治疗前后患者汉密尔顿抑郁量表评分、紧张型头痛VAS评分、紧张型头痛发作频次以及持续时间;中医证候积分。结果自拟中药汤剂治疗组慢性紧张型头痛伴抑郁症疗效高于常规西药盐酸乙哌立松片治疗组,P<0.05。自拟中药汤剂治疗组慢性紧张型头痛缓解的时间、抑郁症状缓解时间短于常规西药盐酸乙哌立松片治疗组,P<0.05。自拟中药汤剂治疗组汉密尔顿抑郁量表评分、紧张型头痛VAS评分、紧张型头痛发作频次以及持续时间均优于常规西药盐酸乙哌立松片治疗组,P<0.05。两组的中医证候积分中,治疗前无显著差异,治疗后自拟中药汤剂治疗组中医证候积分低于常规西药盐酸乙哌立松片治疗组,P<0.05。结论自拟中药汤剂治疗慢性紧张型头痛伴抑郁症效果良好,相对于常规西药盐酸乙哌立松片治疗组患者可更快更好改善病情,缩短治疗时间且可更好减轻抑郁和缓解头痛。
Objective To explore the TCM(traditional Chinese medicine)treatment and clinical prognosis of chronic tension-type headache with depression.Methods Eighty patients with chronic tension-type headache and depression in our hospital from June 2018 to July 2019 were randomly divided into two groups.The patients in the treatment group of routine western medicine eperisone hydrochloride tablet were treated with routine western medicine eperisone hydrochloride tablet,and the patients in the treatment group of self-made Chinese medicine decoction were treated with self-made Chinese medicine decoction.The curative effect of chronic tension-type type headache with depression,the time of relieving chronic tension-type type headache,the time of relieving depression,the Hamilton Depression Scale score,the VAS score of tension-type type headache,the frequency and duration of tensiontype type headache before and after treatment were compared between the two groups.Results The curative effect of chronic tension-type headache with depression in the self-made decoction group was higher than that in the conventional western medicine group(P<0.05).The time of relieving chronic tension-type headache and depressive symptoms in the self-made decoction group was shorter than that in the conventional western medicine eperisone hydrochloride group(P<0.05).The Hamilton Depression Scale score,tension-type headache VAS score,tension-type headache attack frequency and duration of the self-designed Chinese medicine decoction treatment group were better than those of the conventional western medicine eperisone hydrochloride tablet treatment group(P<0.05).There was no significant difference in TCM syndrome score between the two groups before and after treatment.The TCM syndrome score of the self-made TCM decoction treatment group was lower than that of the conventional western medicine eperisone hydrochloride tablet treatment group(P<0.05).Conclusion The self-designed Chinese medicine decoction has a good effect in the treatment of chronic tensi
