摘要
目的:评价正骨推拿治疗椎动脉型颈椎病的临床治疗效果。方法:系统检索中国知网、万方、维普、CBM、Cochrane library、JBI循证卫生保健国际合作中心图书馆、Medline、EM-BASE等数据库中关于正骨推拿治疗椎动脉型颈椎病的随机对照试验,根据Cochrane Handbook(5.1.0)对文献进行质量评价,最后采用RevMan 5.2软件对资料进行分析。结果:共纳入10篇RCT,888例患者。Meta分析结果显示,正骨推拿组与对照组在提高治疗有效率[RR=1.18,95%CI(1.12,1.24),P<0.05],降低颈性眩晕症状与功能评分[MD=2.57,95%CI(1.92,3.21),P<0.05]\增加椎动脉平均血流速度方面[MD=-3.86,95%CI(-5.52,-2.19),P<0.05]存在统计学差异。结论:正骨在改善椎动脉型颈椎病临床症状方面有一定疗效,但由于本次Meta分析纳入研究的文献质量等级较低,分配隐藏及盲法实施性偏倚存在高风险,因此需慎重对待本次系统评价结果,尚待更多高质量、设计规范严谨的临床研究论文加以验证。
Objective：To evaluate the clinical efficacy of spinal manipulation therapy for neck pain with dizziness.Methods：Databases such as the CNKI,WanFang,VIP,CBM,Cochrane library,JBI,Medline,EMBASE were searched to collect randomized controlledtrials（RCTs）on spinal manipulation therapy for neck pain with dizziness.Cochrane Handbook-version 5.1.0 was used to assess trial quality.Meta analysis were conducted for the eligibleRCTs using RevMan 5.2.Results：A total of 10 RCTs involving888 patients were incl-uded.Results of meta analysis showed that there was significant difference between the twogroups in the efficacy rate[RR=1.18,95%CI（1.12,1.24）,P〈0.05],the score of cervical vertigo symptom and function[MD=2.57,95%CI（1.92,3.21）,P〈0.05]and the mean blood flow velocityof vertebral artery[MD=-3.86,95%CI（-5.52,-2.19）,P〈0.05].Conclusion：Spinal manipulation therapy has certain effect on the neck pain with dizziness.However,the results of this meta analysis need to be cautiously considered because the quality of the included studies is limited and the included studies have a high risk of allocation concealment and blinding,Which needs more clinical studies with high quality and strict design for further validation.
