摘要
目的观察分析发散式冲击波结合林氏正骨推拿治疗腰椎间盘突出症的临床效果。方法将126例腰椎间盘突出症患者随机对照试验(RCT)分为两组,对照组63例给予林氏正骨推拿治疗;观察组63例则在对照组基础上给予发散式冲击波治疗,并比较两组的临床效果。结果观察组的总有效率为98.24%明显高于对照组77.78%(χ2=8.2277,P<0.05),两组治疗前下腰痛JOA评分比较,差异无统计学意义(P>0.05);治疗后2周及4周两组下腰痛JOA评分同治疗前都有升高,差异有统计学意义(P<0.05),而观察组JOA评分均明显高于对照组,差异有统计学意义(P<0.05),两组治疗后VAS评分较治疗前均有下降,差异有统计学意义(P<0.05),而观察组VAS评分显著地低于对照组,差异有统计学意义(P<0.05)。结论发散式冲击波(RSWT)结合林氏正骨推拿治疗腰椎间盘突出症具有明显的疗效,创伤小,见效快,能够显著的提高JOA腰痛评分,降低疼痛不适感,提高患者的生活质量。
Objective To observe and analyze the clinical effect of radial shock wave therapy combined with Lin＇s bone setting manipulation in the treatment of lumbar disc herniation（LDH）. Methods One hundred and twenty-six patients with lumbar disc herniation by randomized controlled trial（RCT） were divided into two groups, the 63 cases of control group were given Lin＇s bone setting manipulation treatment, the 63 cases of observation group were given in the control group on the basis of radial shock wave therapy. And the clinical effects of the two groups were compared. Results The total effective rate of the observation group was 98.24%, which was significantly higher than that of the control group（77.78%, χ~2= 8.2277, P〈0.05）. There was no significant difference in JOA score for low back pain between the two groups before and after treatment（P〈0.05）. The JOA score for low back pain in both groups at 2 and 4 weeks were significantly higher than those before treatment（P〈0.05）, while the JOA score in the observation group was significantly higher than that in the control group（P〈0.05）. The VAS scores of both groups decreased after treatment（P〈0.05）, while the VAS score in the observation group was significantly lower than that in the control group（P〈0.05）. Conclusion Radial shock wave therapy combined with Lin＇s bone setting manipulation has obvious curative effect on lumbar disc herniation, which can significantly improve JOA score for low back pain, reduce pain discomfort and improve patient＇s quality of life.
