摘要
目的:系统地评价穴位埋线治疗癫痫的疗效及安全性。方法:计算机检索国内外相关数据库,检索时间为建库至2017年4月15日。纳入穴位埋线与安慰剂、抗癫痫药物(AEDs)对比治疗癫痫的随机对照试验(RCT),采用国际公认的评价标准对纳入试验的质量包括随机化、分配隐藏、盲法、退出与失访进行评价,进而对疗效进行系统综合分析。结果:共计纳入13项RCT(1 291例患者)。8项研究报告了穴位埋线联合AEDs与AEDs的比较,5项研究报告了穴位埋线与AEDs的比较。3项研究表明穴位埋线联合AEDs在改善发作频次方面有优势。11项研究表明穴位埋线在提高显效率方面有优势。仅2项研究涉及不良反应评价,因此不能对穴位埋线的安全性做出结论。结论:与AEDs比较,穴位埋线联合AEDs治疗癫痫在改善癫痫发作频次、提高显效率方面有一定优势,但需要进一步严格设计的临床试验进行证实。
Objective： To systematically assess the curative effect and safety of acupoint catgut embedding on epilepsy. Methods： The domestic and overseas databases were retrieved until April 15, 2017. The randomized controlled trials（RCT） about the comparison in the treatment of epilepsy among acupoint catgut embedding, placebo or AEDs were identified and included. The quality of included RCT, such as randomization, allocation concealment, blinding, quitting and lost to follow-up, was assessed according to internationally recognized criterions, and then the curative effect was analyzed systematically and comprehensively. Results： There were totally 13 RCT included（involving 1 291 patients）. There were 8 studies reported the comparison of acupoint catgut embedding with AEDs and AEDs, and 5 studies reported the comparison of acupoint catgut embedding and AEDs. There were 3 RCT showed that acupoint catgut embedding combined with AEDs had advantages in seizure frenquency, 11 studies showed that acupoint catgut embedding had advantage in improving effective rate, only 2 reported the adverse reactions evaluation, so the safety of acupoint catgut embedding was not obtained affirmation. Conclusion： Compared with AEDs, acupoint catgut embedding combined with AEDs have advantages in epilepsy treatment, but more rigorous designed large-sample clinical trials are needed for further confirmation.
