摘要
目的探讨小切口白内障摘除联合人工晶状体植入术及房角分离术对短眼轴膨胀期白内障患者房角、眼压等影响。方法回顾性分析2016年6月1日至2017年5月31日我院眼科就诊的49例(61只眼)膨胀期白内障合并短眼轴患者,按照手术方式的不同分成标准组及改良组,其中标准组22例(30只眼)行标准小切口白内障摘除联合人工晶状体植入术(以下称为标准组),改良组27例(31只眼)行小切口白内障摘除联合人工晶状体植入术联合房角分离术(以下称为改良组),观察术后1周,1、3、6个月的矫正视力、房角、眼压,术后3、6个月前房深度、视网膜神经纤维层(RNFL)厚度等的改变。结果对两组矫正视力、眼压、前房深度,房角开放距离(AOD)500、AOD750,前房角(ACA)500、ACA750,RNFL颞侧、鼻侧、上方、下方行重复测量方差分析,两组间矫正视力、眼压、AOD500、AOD750、ACA500、ACA750,组间差异P值均<0.05,组间差异有统计学意义,进一步行两独立样本t检验分析两组相同时间点统计学差异,术后1周、术后1、3、6个月两组间眼压、AOD500、AOD750、ACA500、ACA750组间差异,P值均<0.05,差别均有统计学意义。两组间矫正视力、眼压、AOD500、AOD750、ACA500、ACA750、RNFL颞侧、前房深度进行时间差异检验,P值均<0.05,时间差异有统计学意义,进一步用LSD-t检验分析存在时间差异的指标。结论短眼轴膨胀期白内障患者行小切口白内障摘除联合人工晶状体植入术及房角分离术,术后房角开放明显增大,眼压明显减低,对降低白内障术后并发高眼压及青光眼有很好的疗效。
Objective To explore the effect of anterior chamber angle and intraocular pressure etc,by small incision cataract surgery combined with intraocular lens implantation and goniosynechialysis for intumescent cataract with short eye axis. Methods From June 2016 to May 2017,49 patients（ 61 eyes） with intumescent cataract and short eye axis,were randomly divided into standard group and the modified group,the standard group had 22 patients（ 30 eyes） by small incision cataract surgery with intraocular lens implantation（ hereinafter referred to as the standard group）,the modified group had 27 patients（ 31 eyes） by small incision cataract surgery combined with intraocular lens implantation and goniosynechialysis（ hereinafter referred to as the modified group）. the change of the intraocular pressure、Anterior chamber angle、the visual acuity,after surgery 1 week、1、3 、6 months. and anterior chamber depth、Retinal nerve fiber layer thickness after surgery 3 monthsand 6 months had observed in both teams. Results Repeated measurement variance of analysis was performed between the two groups in corrected visual acuity,IOP,anterior chamber depth,AOD500,AOD750,ACA500,ACD750,RNFL temporal side,RNFL nasal side,superior RNFL,and inferior RNFL. There were statistically significant differences between groups in terms of corrected visual acuity、IOP、AOD500、AOD750、ACA500 and ACD750. Two independent samples t-test analysis indicators between two group are significant differences in IOP、AOD500、AOD750、ACA500、ACA750,at the time after opration 1 week、3 months、6 months,all the P value was 〈0. 05. There were statistically significant differences between difference time in incorrected visual acuity、IOP、AOD500、AOD750、ACA500、ACD750 and RNFL on the temporal side、anterior chamber depth. Further analysis those parameter in difference time by LSD-t test. Conclusions It has good curative effect such as increased anterior chambe and decreased intraocular pressure significantly for the intumes
