摘要
目的探讨小切口白内障囊外摘除术和超声乳化吸出术治疗硬核白内障的临床疗效和并发症。方法选取2011年1月-2014年1月我院收治的硬核白内障患者130例（178眼）行回顾性对照研究,分为观察组和对照组,观察组58例（76眼）,行小切口白内障囊外摘除术及人工晶体植入术;对照组72例（102眼）,行超声乳化吸出术及人工晶体植入术,观察两组治疗后的视力、散光以及并发症发生情况。结果术后观察组患者视力恢复较对照组快,矫正视力显著优于对照组,差异有统计学意义（P〈0.05）。术后观察组患者角膜散光度显著小于对照组,差异有统计学意义（P〈0.05）。术后观察组角膜水肿、纤维素样渗出、人工晶体表面色素沉着、巩膜粘连和黄斑囊样水肿等并发症发生率显著低于对照组,差异有统计学意义（P〈0.05）。结论对硬核白内障患者实施小切口白内障囊外摘除术及人工晶状体植入术,患者术后视力恢复情况好,角膜散光程度较小,术后并发症发生率较低,值得在临床推广应用。
Objective To investigate the clinical efficacy and complications of small incision extracapsular cataract extraction and phacoemulsification（ PHACO） on hard nucleus cataract. Methods 130 cases（ 178 eyes） of hard nucleus cataract from 2011 January - 2014 January in our hospital were selected,and were divided into observation group and control group. The observation group included 58 cases（ 76 eyes） treated with small incision extracapsular cataract extraction and intraocular lens implantation. The control group included 72 cases（ 102 eyes） treated with PHACO and implantation of artificial lens. The vision,astigmatism and complications of two groups were observed. Results The corrected visual acuity and visual acuity in observation group was better than that of control group,and the uncorrected visual acuity in observation group was higher than that of control group. The differences between two groups were statistically significant（ P〈0. 05）. Corneal astigmatism of observation group was less than that of the control group after operation（ P〈0. 05）. The corneal edema,fibrous exudation,artificial crystal surface pigmentation,sclera adhesion and macular edema in the observation group after operation were lower than that of control group（ P〈0. 05）. Conclusion The implementation of small incision extracapsular cataract extraction and intraocular lens implantation in cataract patients has good corrected visual acuity recovery,less corneal astigmatism,and low postoperative complication rate. And it is worthy of clinical application.
