摘要
目的探讨品管圈对儿童静脉留置针72 h安全留置率的干预效果,为临床研究提供参考。方法选取2016年1—12月在我院儿科就诊并行静脉留置针患儿共380例,按照品管圈实施时间分为实施前和实施后。实施前仅给予常规干预,实施后建立品管圈,采取PDCA循环管理模式确立"静脉留置针72 h安全留置率"为活动主题,现状调查后给予原因及对策分析。比较两组安全留置率、相关拔管原因。结果实施后72 h安全留置率明显高于实施前,差异有统计学意义(P<0.05);实施后留置未达72 h拔管发生相关原因中导管脱出、局部肿胀、导管堵塞因素明显低于实施前(P均<0.05),而穿刺点渗血和留置针泡水与实施前比较差异无统计学意义(P>0.05)。结论品管圈的运用能够明显提高儿童静脉留置针72 h安全留置率,显著降低拔管发生率,提升维护质量,具有临床推广与应用价值。
Objective To explore the effect of quality control circle on the 72 hours safety indwelling rate of venous indwelling needle in children,and provide reference for clinical research. Methods A total of 380 cases of children with venous indwelling needles in our hospital from January 2016 to December 2016 were selected as research subjects. They were divided into pre-implementation group and post-implementation group according to the time of quality control circle implementation. Before the implementation,only routine intervention was performed. After the implementation,the quality control circle was established. The PDCA circulation management mode was adopted to establish the ＂ safety indwelling rate of the 72 hours venous indwelling needle＂ as the theme of the activity. After the investigation,the reasons and countermeasures were analyzed. The safety indwelling rate of the two groups and the reasons for related extubation were compared in the two groups. Results The safety indwelling rate 72 hours after the implementation of quality control circle was significantly higher than that before the implementation,and the difference was statistically significant（P 〈 0. 05）. After the implementation,regarding the factors related to catheter extubation of venous indwelling needle that less than 72 hours,the catheter extubation,local swelling,and catheter obstruction factors were significantly lower than that before the implementation.（all P 〈 0. 05）. There was no significant difference in puncture point bleeding and soak in water of venous indwelling needle before and after implementation（P 〉 0. 05）. Conclusion The use of quality control circle can significantly improve the 72 hours safety indwelling rate of venous indwelling needle in children. It can reduce the incidence of extubation significantly. Meanwhile,it can improve the quality of maintenance,and has clinical promotion and application value.
