摘要
目的探讨超声心动图在小儿川崎病冠状动脉病变诊断及随访中的应用价值。方法对84例临床诊断川崎病的患儿进行超声心动图检查,通过二维超声心动图重点观察冠状动脉,判断是否存在冠状动脉瘤、血栓和冠状动脉狭窄,常规测量冠状动脉内径,对伴有冠状动脉病变者至少测量3次,取其平均值,并进行跟踪随访,随访6个月至5年。结果 84例患儿中有43例出现冠状动脉病变(51%),其中左冠状动脉扩张16例(30%),右冠状动脉扩张10例(23%),双侧冠状动脉扩张13例(23%),巨大冠状动脉瘤4例(9%),比较治疗前后左右冠状动脉内径明显缩小,差异有统计学意义(P<0.05)。结论超声心动图作为一种无创的影像学检查方法,可以实时观察冠状动脉的病变程度及转归,有助于早期诊断和长期随访,对判断病情指导治疗、估计预后具有重要作用。
Objective To investigate the application value of echocardiography in the diagnosis of coronary arterial lesion in children with Kawasaki and in the follow-up. Methods Echocardiographic examinations were performed in the 87 children patients with Kawasaki disease by clinical diagnosis. Their coronary arteries were mainly observed through cross-sectional echocardiography to judge whether coronary aneurysms, thromboses or coronary stenoses existed. Routine surveying was conducted of coronary artery diameter. Such surveying was done in the pa-tients with coronary arterial lesion at least three times, and the average was taken. They were followed-up for six months to five years. Results In 84 children, there were 43 cases of coronary arterial lesion(51%), including 16 cases with left coronary artery dilation(30%), 10 cases with right coronary artery dilation(23%), 13 cases with bothside coronary artery dilations(23%) and 4 cases with giant coronary aneurysms(9%). The comparisons of left and right coronary artery diameters before and after treatment showed that they clearly shrank. The difference was statis-tically meaningful(P〈0.05). Conclusion As a method of non-invasive imageological examination, echocardiography makes it possible to conduct the real-time observation of the degree of arterial lesion and prognosis, which is helpful for early diagnosis and long-term follow-up and plays an important role in judging the state of the disease, guiding treatment and estimating prognosis.
