摘要
目的探讨慢性阻塞性肺疾病(COPD)呼吸道病毒感染患者的肺功能、血清免疫球蛋白轻链和炎性细胞因子水平的相关性。方法回顾性分析我院接受治疗的慢性阻塞性肺疾病患者的临床资料,根据其是否合并呼吸道病毒感染将其分为COPD(n=40)组和COPD合并呼吸道病毒感染组(n=35)。比较2组患者肺功能[6 min步行试验(6MWT)、用力肺结量(FVC)、第1秒用力呼气量(FEV1)、FEV1/FVC、最大呼气中段平均流速(MMEF)、呼气值流速(PEF)]、血清免疫球蛋白轻链(κFLC、λFLC)和炎症细胞因子[白介素-18(IL-18)、IL-6、超敏C-反应蛋白(hsCRP)]水平的差异,分析COPD合并呼吸道病毒感染患者血清免疫球蛋白轻链水平与肺功能和炎性细胞因子水平的相关性。结果合并呼吸道病毒感染组患者的6MWT、FVC、FEV1和FEV1/FVC等肺功能指标均较COPD组低,差异有统计学意义(P<0.05);合并呼吸道病毒感染组患者的κFLC、λFLC、IL-18、IL-6和hs-CRP水平均高于COPD组(P<0.05);COPD合并呼吸道病毒感染患者的κFLC和λFLC水平与hs-CRP呈正相关(r值分别为0.021、0.017,P<0.05),与FVC、FEV1、FEV1/FVC和MMEF水平负相关(r值分别为-0.352、-0.405、-0.387、-0.412、-0.397、-0.412、-0.395、-0.402,P<0.05)。结论 COPD合并呼吸道病毒感染患者的血清免疫球蛋白轻链水平较高,且与患者肺功能指标负相关,与炎性细胞因子水平正相关。
Objective To investigate the correlation between pulmonary function,serum immunoglobulin light chain and inflammatory cytokine levels in patients with chronic obstructive pulmonary disease(COPD). Methods The clinical data of patients with chronic obstructive pulmonary disease who were admited and treated in our hospital from May 2014 to May2016 were retrospectively analyzed. According to whether the patients had respiratory tract virus infection,these patients were divided into COPD group and COPD + infection group. The pulmonary function including 6 MWT,FVC,FEV1,FEV1/FVC,MMEF and PEF,serum immunoglobulin light chain protein(κFLC,λFLC) and the levels of inflammatory cytokines including IL-18,IL-6,hs-CRP were observed and comapred between two groups,and the correlation between pulmonary function,serum immunoglobulin light chain and inflammatory cytokine levels in patients with COPD complicated by respiratory tract virus infection were analyzed. Results The levels of 6 MWT,FVC,FEV1 and FEV1/FVC in COPD + infection group were significantly lower than those in COPD group(P〈 0. 05),however,the levels of κFLC,λFLC,IL-18,IL-6 and hs-CRP in COPD + infection group were significantly higher than those in COPD group(P〈 0. 05). Moreover the levels ofκFLC andλFLC were positively correlated with those of hs-CRP in patients with COPD complicated by respiratory virus infection,however,which were negatively correlated with those of FVC,FEV1,FEV1/FVC and MMEF(P〈 0. 05). Conclusion The serum immunoglobulin light chain levels(κFLC,λFLC) are abnormally highly expressed in patients with COPD,moreover,which are closely correlated with the lung function indexes and the level of inflammatory cytokines.
