摘要
目的探讨河北省农村地区精神障碍患者的就诊情况及影响因素。方法在河北省农村地区进行抽样调查，以《精神障碍诊断与统计手册》（DSM．1V）诊断标准，以《DSM-Ⅳ轴Ⅰ障碍用临床定式检查》（SCm—I/P）作为诊断工具。筛选出I58I例农村地区患者进行自制的精神卫生服务利用问卷调查，由调查员询问调查对象并填写“精神障碍患者求医行为调查表”（自编）。结果有13．2％（208／1581）的患者曾就诊，女性患者就诊率（14．9％）显著高于男性患者（11．3％）。Logistic回归分析结果显示，精神障碍患者就诊情况的影响因素有是否参加新型农村合作医疗（OR=2．116）、年龄（OR=1．882）、是否接受疾病知识宣传（OR=5．186）、就业状况（OR=0．522）和是否为低保户（OR=2．863）。结论河北省农村精神疾病患者精神疾病知识缺乏，卫生服务利用率很低，影响卫生服务利用的因素主要是年龄、就业状况、疾病知识宣传、是否参加新农合及是否为低保户等。
Objective To explore the status of accessing medical service of patients with mental disorders in rural areas and its influencing factors. Methods A sampling survey in rural areas of Hebei Province was carried out based on the DSM- Ⅳ criteria , using the ＂DSM- Ⅳ axis Ⅰ disorder in clinical interview check ＂ （SCID- I /P） as a diagnostic tool. A total of 1 581 patients were selected in rural areas to take the self-made questionnaire of mental health services. All of the objects were interviewed with ＂Questionnaire on Behaviors of Help Seeking of Patients with Mental Disorders＂ （self-compiled） by investigators. Results At last, 208 （13.2%） patients visited hospitals, and the rate of female patients （14.9%） was significantly higher than that of male patients （11.3%）. The results of Binary Logistic regression showed that the influencing factors of the status of accessing medical service were whether they have participated the new rural cooperative medical care system （OR=2.116）, age （OR=1.882）, whether they have received the information of disease （OR=5.186）, their employment situation （0R=0.522） and whether they are low-income residents （0R=2.863）. Conclusions The utilization of mental health services is considerably low among rural patients with mental disorders who lack of the knowledge of mental disease in Hebei. The main factors influencing health services utilization are gender, age, marital status, employment status, disease information propaganda, clinic distance, whether they had participated in the new rural cooperative medical care system and whether they are low-income residents.
