摘要
目的研究益气养阴中药方联合胰岛素皮下泵强化治疗新诊断2型糖尿病(气阴两虚证)患者的疗效及其对炎症因子的影响。方法将我院2010年12月~2016年10月收治的110例新诊断2型糖尿病患者(气阴两虚证)随机分为治疗组(n=54)和对照组(n=56),对照组予胰岛素泵强化治疗,治疗组在此基础上加用益气养阴中药方,疗程2周,评估患者的中医症候积分,血糖(包括空腹血糖及餐后2 h血糖)、空腹及餐后2 h血胰岛素水平、胰岛素敏感指数(ISI)、血脂、C-反应蛋白(CRP)、肿瘤坏死因子-α(TNF-α)、白介素-6(IL-6)等变化。结果经治疗后,与对照组比较,治疗组患者的中医证候明显改善;治疗组患者血糖、总胆固醇(TG)、甘油三酯(TC)、低密度脂蛋白胆固醇(LDL-C)及炎症因子(CRP、TNF-α、IL-6)较治疗前均有明显下降(P<0.05);胰岛素敏感指数较治疗前改善。结论益气养阴中药方联合胰岛素皮下泵治疗可明显改善新诊断2型糖尿病(气阴两虚证)患者的临床症状,降低血糖血脂,改善胰岛素敏感性及降低体内炎症因子水平。
Objective To study the effect of traditional Chinese medicine combined with insulin subcutaneous pump in the intensive treatment of newly diagnosed type 2 diabetes mellitus （Deficiency of Qi and Yin） and its effect on inflammatory factors. Methods 110 patients with newly diagnosed type 2 diabetes （Qi and Yin deficiency） admitted in our hospital from December 2010 to October 2016 were randomly divided into treatment group（n=54） and control group（n= 56）. The control group was given insulin pump intensive treatment. The treatment group was supplemented with Yiqi Yangyin prescription on the basis of this treatment. The course of treatment was 2 weeks. The changes of patient＇s TCM symptom scores, blood glucose （including fasting blood glucose and 2 hours postprandial blood glucose）, and fasting and 2 hours postprandial blood insulin levels, insulin sensitivity index（ISI）, blood lipids, C-reactive protein（CRP）, tumor necrosis faetor-α（TNF-α） and interleukin-6 （IL-6） were evaluated. Results After treatment, the TCM syndromes of the patients in the treatment group improved significantly compared with those of the control group. The blood glucose, total cholesterol （TG）, triglyceride （TC）, low-density lipoprotein cholesterol （LDL-C） and Inflammatory factors（CRP, TNF-α, IL-6） in the treatment group were significantly lower than those before treatment （P〈0.05）. And insulin sensitivity index in the treatment group was improved compared with that before treatment. Conclusion The combination of Yiqi Yangyin traditional Chinese medicine and insulin subcutaneous pump can obviously improve the clinical symptoms of newly diagnosed type 2 diabetes mellitus （Qi and Yin deficiency syndrome）, reduce blood glucose and blood lipids, improve insulin sensitivity and reduce the level of inflammatory factors in the body.
