胰岛素泵持续皮下注射胰岛素对1型糖尿病酮症酸中毒患儿机体代谢紊乱纠正效果研究 被引量:30

摘要 目的:探讨胰岛素持续皮下输注(CSII)胰岛素对1型糖尿病酮症酸中毒(DKA)患儿机体代谢紊乱纠正效果。方法:将94例1型DKA患儿随机分为对照组和观察组,每组各47例。对照组患儿接受胰岛素多次皮下注射(MSII)治疗,观察组患儿接受CSII治疗。比较两组患儿的血糖达标时间、DKA纠正时间、尿酮转阴时间、pH值纠正时间、住院时间及低血糖发生率,同时比较治疗前后的糖尿病相关指标和血细胞因子表达水平。结果:观察组患儿的血糖达标时间、DKA纠正时间、尿酮转阴时间、pH值纠正时间、每日胰岛素用量、住院时间及低血糖发生率均明显低于对照组(P<0.05);治疗后,观察组的CO_2-CP和C肽水平均明显高于对照组(P<0.01),HbAlc和AG水平均明显低于对照组(P<0.01),MIF和MCP-1表达水平均明显低于对照组(P<0.01)。结论:胰岛素泵持续皮下注射胰岛素能有效纠正1型DKA患儿机体代谢紊乱,促进中毒症状消退,抑制血细胞因子表达,且治疗安全性较高。 Objective:To study the effect of continuous subcutaneous insulin infusion(CSII)insulin on type I diabetic ketoacidosis(DKA)in children with metabolic disorders correction effect.Methods:The subjects were 94 cases of I type DKA children randomly divided into control group and observation group.47 patients in each group.The control group was treated with insulin subcutaneous injection(MSII)treatment,observation group were treated with CSII.compared the two groups of the blood glucose time,DKA correction time,urine ketone clearance time,pH correction time,Hospitalization time and incidence of hypoglycemia,and compared before and after treatment of diabetes related indicators and the expression of blood cytokine levels.Results:The observation group of the blood glucose time,DKA correct time,urine ketone clearance time,pH correction time,daily insulin dosage,the time and the incidence of hypoglycemia was significantly lower that in than control group(P〈0.05);after treatment,the observation group of the CO_2-CP and C peptide levels were significantly higher than those in the control group(P〈0.01),HbAlc and AG were significantly lower than those in control group(P〈0.01),the expression level of MIF and MCP-1 were significantly lower than those in the control group(P〈0.01).Conclusion:The insulin pump continuous subcutaneous insulin injection can effectively correct the metabolism of I in children with DKA disorder,promote poisoning symptoms subsided,blood cytokine inhibition,and treatment of high safety.
