超声引导短轴平面外桡动脉穿刺在老年骨折患者桡动脉穿刺置管中的应用效果 被引量:10

Application of ultrasound-guided short axis/out-of-plane approaches technique in the management of arterial puncture in elderly patients with fracture

摘要 目的观察超声引导短轴平面外穿刺置管技术在老年骨折患者桡动脉置管中的应用效果。方法限期行手术治疗老年创伤患者160例,其中男42例,女118例,ASAⅡ~Ⅳ级,采用随机数字表法,将患者分为2组:结合超声显影中位线定标,短轴平面外穿刺组(S组,80例)和超声引导长轴平面内穿刺组(L组,80例)。两组患者动脉穿刺前采用1%利多卡因局部麻醉。记录首次穿刺成功率、穿刺失败率、穿刺置管时间及并发症。结果 S组桡动脉穿刺成功率高(78. 8%vs. 62. 5%,P <0. 05); S组动脉穿刺置管时间短(29. 6s±13. 5s vs. 45. 3s±17. 3s,P <0. 05)。结论超声显影中位线定标短轴平面外穿刺技术成功率高,操作时间短。 Objective To observe the effect of ultrasound-guided short axis/out-of-plane ap- proaches on radial artery puncture in elderly patients with fracture. Methods One hundred and sixty elderly patients with fracture, 42 males and 118 females included, ASA physical status Ⅱ~Ⅳ, were randomly divided into 2 groups: Combined with median location of uhrasound, short axis out-of-plane external puncture group (group S, 80 cases ) and ultrasound guided long axis plane puncture group (group L, 80 cases). The radial artery cannulation was performed after local infiltration anesthesia. The success and failure rate of first attempt, time of puncture catheter and complications were recorded. Results Compared with group L, the success rate of radial artery puncture in group S was higher (78.8% vs. 62.5% , P 〈0. 05) ;the time of arterial puncture was shorter in group S (29.6 ±13.5s vs. 45.3 ± 17.3s, P 〈 0. 05 ). Conclusion The technique of short axis plane external puncture has higher success rate and shorter operation time.
