摘要
目的探讨鼻内窥镜下低温等离子射频消融术对鼾症患儿应激反应及通气功能的影响。方法随机选取2014年5月—2017年1月在该院治疗的鼾症患儿100例作为研究对象,随机分为观察组和对照组各50例,观察组给予鼻内窥镜下低温等离子射频消融术,对照组给予腺样体刮除术以及扁桃体剥离术;比较两组患儿的手术情况、应激反应程度、通气功能指标。结果观察组的手术时间、术后疼痛时间、症状缓解时间及术中出血量分别为(23.31±4.26)min、(2.33±0.29)d、(7.13±0.72)d、(13.34±2.66)m L均明显低于对照组的(34.74±4.58)min、(3.71±0.41)d、(9.81±1.15)d、(31.78±4.98)m L(P<0.05);观察组术后24 h的血清MDA为(7.93±1.01)mol/L明显低于对照组的(10.85±1.72)mol/L,但其血清SOD、GSH含量分别为(69.94±9.33)、(79.73±10.21)mol/L,明显高于对照组的(54.21±7.17)、(61.42±7.13)mol/L(P<0.05);观察组术后6个月的SLT90%、LAT分别为(9.93±1.54)%、(17.77±3.99)s明显低于对照组的(13.73±2.33)%、(26.51±5.13)s,但LSa O2为(87.37±6.23)s明显高于对照组的(76.58±8.53)s(P<0.05)。结论鼻内窥镜下低温等离子射频消融术在治疗鼾症患儿上有手术创伤小、疗效好等优点,值得推广应用。
Objective This paper tries to study the effect of nasal endoscopic low temperature plasma radiofrequency ablation on stress response and ventilation function in children with snoring. Methods 100 cases of snoring children who were random selected from May 2014 to January 2017 were selected and randomly divided into the observation group(50 case)and the control group(50 case). The observation group received nasal endoscopic low temperature plasma radiofrequency ablation, the control group received adenoidectomy curettage combined with tonsillectomy. Then the surgery condition, stress response, ventilation function were compared between the two groups. Results The operative time, postoperative pain time,symptom relief time, and intraoperative blood loss in the observation group were(23.31±4.26) min,(2.33±0.29) days,(7.13±0.72) days, and(13.34 ±2.66)m L, respectively, significantly lower than that of(34.74 ±4.58) min,(3.71 ±0.41) days,(9.81 ±1.15) days,(31.78±4.98)m L in the control group(P〈0.05); serum MDA in the observation group at 24 hours after surgery was(7.93±1.01) mol/L was significantly lower than that in the control group(10.85±1.72) mol/L, but the serum SOD and GSH levels were(69.94±9.33) mol/L and(79.73±10.21) mol/L were significantly higher than the control of(54.21±7.17) mol/L and(61.42±7.13) mol/L(P〈0.05); SLT 90% and LAT in the observation group 6 months after operation were(9.93 ±1.54)%,(17.77±3.99)s were significantly lower than(13.73±2.33)% and(26.51±5.13) s in the control group, but LSa O2 was(87.37±6.23)s higher than that in the control group(76.58±8.53)s(P〈0.05). Conclusion Low-temperature plasma radiofrequency ablation under nasal endoscope has advantages of small surgical trauma, good curative effect, etc., and is worthy of popularization and application.
