摘要
目的:探讨前列地尔注射液联合安络化纤丸治疗失代偿期乙型肝炎肝硬化的疗效及对肝纤维化指标的影响。方法:选取2015年7月至2017年6月我院门诊收治的138例失代偿期乙型肝炎肝硬化患者,按照随机区组设计法将患者分为观察组(n=69)和对照组(n=69)。观察组患者采用前列地尔注射液联合安络化纤丸治疗,对照组患者采用前列地尔注射液治疗。观察两组患者肝功能、肝纤维化、肝脾超声影像学指标、Child-pugh积分等变化及不良反应发生率。结果:治疗后观察组患者ALT、AST、TBil、DBil、HA、LN、PCⅢ及Ⅳ-C指标均显著低于对照组(P<0.05);观察组患者门静脉内径、脾静脉内径、脾脏厚度均显著低于对照组(P<0.05);观察组患者Child-pugh积分显著低于对照组(P<0.05);对照组患者不良反应发生率(15.9%)略高于观察组(8.7%),但两组比较差异无统计学意义(P>0.05)。结论:前列地尔注射液联合安络化纤丸可有效改善失代偿期乙型肝炎肝硬化患者的肝功能和肝纤维化指标,有效抑制肝硬化进展。
Objective：To investigate the efficacy of alprostadil injection combined with anluohuaxian capsule on decompensated hepatitis B cirrhosis and its influence on liver fibrosis index. Methods：From July 2015 to June 2017,138 patients with decompensated hepatitis B cir- rhosis in our hospital were selected as the subjects. All the patients were divided into the observation group and the control group according to the random grouping method ,69 cases in each group. The observation group was treated with alprostadil injection combined with anluohuaxian capsule, the control group was treated with alprostadil injection. The liver function index,liver fibrosis index, ultrasonic imaging index, Child - pugh integral and adverse reaction rate were compared between the two groups. Results ： After treatment, the indexes of ALT, AST, TBIL, DBIL in the observation group were significantly lower than those in the control group（P 〈 0.05）. The indexes of HA,LN, PC, Ⅲ and IV - C in the observation group were significantly lower than those in the control group （ P 〈 0.05 ）. The diameter of portal vein, the diameter of splenic vein and the thickness of spleen in the observation group were significantly lower than those in the control group （ P 〈 0.05 ）. The Child - pugh score of the observation group was significantly lower than that of the control group （ P 〈 0.05 ）. The adverse reaction rate of control group （ 15.9% ） was slightly higher than that of control group（8.69% ）, but there was no statistical difference between the two groups（P 〉 0.05 ）. Conclusion：Alprostadil injection compared with anluohuaxian capsule can effectively improve the liver function and liver fibrosis index in pa- tients with decompensated liver cirrhosis, and effectively inhibit the process of cirrhosis.
