摘要
目的分析不同切口方式白内障超声乳化联合人工晶体植入术后干眼症的临床效果。方法随机抽取76例吉林医药学院附属四六五医院自2016年10月—2017年11月收治的行白内障超声乳化联合人工晶体植入术患者,根据不同的切口方式将其分为对照组(n=38)、治疗组(n=38)。对照组采用透明角膜切口白内障超声乳化联合人工晶体植入术治疗,治疗组应用巩膜隧道切口白内障超声乳化联合人工晶体植入术治疗,分析两组术后临床症状评分、临床指标、术后干眼症发生率。结果临床症状评分、术后干眼症发生率对比:治疗组异物感(0.6±0.4)分、干涩感(0.5±0.3)分、烧灼感(0.5±0.3)分、眼红(0.7±0.3)分、分泌物黏附睫毛(0.6±0.4)分、晨起眼睑黏滞低(0.7±0.4)分、13.2%,低于对照组(1.7±0.4)分、(1.5±0.3)分、(1.6±0.5)分、(1.8±0.4)分、(1.6±0.4)分、(1.5±0.5)分、42.1%(t=11.987、14.530、11.629、13.562、10.897、7.702、χ2=7.962,P<0.05);临床指标对比:治疗组角膜荧光染色结果(3.4±0.3)分、泪膜破裂时间(5.5±1.8)s、基础泪液分泌量(11.5±3.0)mm/5 min优于对照组(5.9±0.4)分、(3.5±1.9)s、(6.5±2.6)mm/5 min(t=30.822、4.711、7.764,P<0.05)。结论巩膜隧道切口白内障超声乳化联合人工晶体植入术相对于透明角膜切口白内障超声乳化联合人工晶体植入术,治疗效果更高,其对改善患者临床指标、术后临床症状、降低术后干眼症发生率具有重要作用,临床应用价值较高,值得推广。
Objective To analyze the clinical effect of different incision ways in treatment of dry eye syndrome after the cataract phacoemulsification combined with artificial lens implantation. Methods 76 cases of patients after the cataract phacoemulsification combined with artificial lens implantation admitted and treated in 465 ffiliated hospital of Jilin Medical college from October 2016 to November 2017 were selected and divided into two groups according to different incision ways with 38 case in each, the control group used the cataract phacoemulsification combined with artificial lens implantation at clear corneal incisions, while the treatment group used the cataract phacoemulsification combined with artificial lens implantation at sclera tunnel incision, and the postoperative clinical symptoms scores, clinical indicators and incidence rate of postoperative dry eye syndrome were compared between the two groups. Results In terms of clinical symptoms scores and incidence rate of postoperative dry eye syndrome, the foreign body sensation, sensation of dryness, burning sensation, red eye, exudates adhered to eyelashes and lower eyelid viscosity in the morning in the treatment group were lower than those in the control group, [(0.6±0.4)points,(0.5±0.3) points,(0.5±0.3) points,(0.7±0.3)points(0.6 ±0.4) points,(0.7 ±0.4)points, 13.2% vs(1.7 ±0.4)points,(1.5 ±0.3)points,(1.6 ±0.5)points,(1.8 ±0.4)points,(1.6±0.4)points,(1.5±0.5)points, 42.1%(t=11.987,14.530,11.629,13.562,10.897,7.702,χ2=7.962,P〈0.05), in terms of clinical indicators, the corneal fluorescence staining results, tear film rupture time, basal tear secretion in the treatment group were better than those in the control group, [(3.4±0.3)points,(5.5±1.8)s,(11.5±3.0)mm/5 min vs(5.9±0.4)points,(3.5±1.9)s,(6.5±2.6)mm/5 min(t= 30.822,4.711,7.764,P〈0.05).
