摘要 目的:研究磁共振胰胆管成像(MRCP)结合Gd-E0B-DTPA增强磁共振胆管成像(EOBMRC)对胆管系统结构的显示。方法:回顾性分析46例患者MRI胆管系统图像,所有患者均同时行MRCP扫描和Gd-EOB-DTPA肝脏动态增强扫描、延迟期及肝胆特异期扫描。分析MRCP及EOBMRC胆管系统结构显示情况。结果:MRCP对胆囊管的显示受胃肠高信号背景干扰较大,MRC胆道显示受肝功能影响。胆道内造影剂正常排泄者,MRCP及MRC对肝总管、胆总管及肝内一级、二级胆管显示无明显差异;胆道系统扩张者,MRCP能够显示更细的肝内胆管分支。结论:MRCP与EOB-MRC均可以清晰显示胆管系统结构,联合应用可进一步了解肝功能。 Objective: To investigate the evaluation of MRCP combined with Gd-EOB-DTPA enhenced liver MRI in the structure of bile duct system, Methods: 46 cases of MRI cholangiographies of bile duct system were retrospectively analyzed, and all patients underwent MRCP and Gd-EOB-DTPA dynamic enhenced liver MRI scan, delayed and hepatobiliary specific scan. The display of MRCP and EOB-MRC bile duct system structures were analyzed. Results: MRCP was strongly disturbed by the background of high gastrointestinal signals in the display of cystic duct and ampullary bile duct, and EOB-MRC was mainly affected by the liver fuctions in the display of bile duct. For patients with normal excretion of contrast medium in biliary tract, there were no significant differences for MRCP and EOB-MRC in the display of common hepatic duct, common bile duct, intrahepatic grade I and II bile duct ; for patients with dilatation of biliary system, MRCP could show finer intrahepatic bile duct branches. Conclusion: Both MRCP and EOB-MRC can clearly show the biliary system structures, and the conbined application will be able to further understand the liver functions.
