摘要
目的 探讨糖尿病神经性膀胱(DNB)患者血清叶酸(FA)、维生素B_(12)(VitB_(12))、血浆同型半胱氨酸(Hcy)及尿动力学指标的变化及其临床意义。方法 选取许昌市中心医院内分泌科收治的T2DM患者251例,其中合并DNB患者86例(DNB组)、单纯T2DM患者165例(T2DM组),比较两组FA、VitB_(12)、Hcy及尿动力学指标。结果 DNB组FPG、2hPG、HbA1c、尿微量白蛋白(UMA)、膀胱最大测压容量(VMBC)、Scr、BUN、Hcy高于T2DM组(P<0.01);FIns、FA、VitB_(12)、最大尿流率(MFR)、最大尿流率时逼尿及压力(PQmax)、膀胱顺应性(BC)低于T2DM组(P<0.01)。结论 DNB患者血清FA、VitB_(12)、Hcy及尿动力学指标均发生改变。
Objective To investigate the changes and the clinical significance of serum folic acid（FA）,vitamin B12（VitB12）,plasma homocysteine（Hcy）and urinary dynamics in patients with diabetic neurogenic bladder（DNB）. Methods 251 inpatients with type 2 diabetes mellitus（T2 DM）were divided into DNB group（n=86）and simple T2 DM group（n=165）.The circulating levels of FA,VitB12,Hcy and urodynamic indexes were detected and compared between two groups. Results The levels of FPG,2 hPG,HbA1 c,UMA,Scr,BUN and Hcy were higher in DNB group than those in T2 DM group（P〈0.05）.The levels of FIns,FRA and VitB12 were lower in DNB group than those in T2 DM group（P〈0.05）.DNB group had significantly lower MFR [（10.7±2.9）vs（25.8±3.3）ml/s],PQ max [（27.5±3.9）vs（51.8±6.4）cm H2 O]and BC [（54.8±13.8）vs（348.9±130.2）ml/kPa]than T2 DM group（P〈0.01）.VMBC in DNB patients was higher than that in non-DNB patients（P〈0.01）. Conclusion The circulating levels of FA,VitB12,Hcy and urodynamic indexes were significantly changed in DNB patients,which had a certain value for the diagnosis and treatment.
