摘要
目的:介绍用三轮德尔菲法形成《儿科阿奇霉素注射使用的快速建议指南》的推荐意见的方法与流程,以期为指南的制订提供方法学支持。方法:秘书组将27个临床问题形成的24条草拟的推荐意见的证据情况汇总后,在推荐意见形成会上呈现给专家组,专家组结合临床实践经验、证据质量、患者(及家属)意愿和价值观及经济学分析并权衡利弊后,通过三轮德尔菲法和GRADE网格法打分,根据共识规则形成指南推荐意见,该过程通过积极系数和协调系数进行质量控制,最终结果交由指导委员会审核通过。结果:共15位专家参与打分,第一轮有16条推荐意见达成共识,第二轮收到23条匿名反馈意见,第三轮有2条达成共识,4条不在本指南中形成推荐,2条进行合并,最终共形成18条推荐意见(9条强推荐,9条弱推荐),共识率达75%,积极系数均为100%,各推荐意见协调系数差异较大,最小为0,最大为50.44%。结论:本文详细介绍和总结了用三轮德尔菲法形成指南推荐意见的方法与流程,为指南推荐意见的形成提供了方法学支持。
OBJECTIVE To introduce the method and process of the three-round Delphi method to develop recommendations included in the Rapid Advice Guideline for Intravenous Azithromycin in Pediatrics,in order to provide methodological supports for the development of the guideline.METHODS After compiled by the secretary group,the evidence of twenty-four drafted recommendations from twenty-seven clinical problems were presented to the guideline panel at the suggestion meeting.Combing with the clinical practice experiences,the quality of evidence,the values and preferences of the patients（or their dependents）,economic analysis and balance between benifits and risk,the recommendations were finally developed based on the consensus rules after the three-round Delphi method and GRADE grid method scoring and through the enthusiasm indexes and conformity indexes for quality control.The final results would be approved by the steering committee.RESULTS A total of fifteen experts were involved in the scoring,sixteen recommendations reached consensus for the first round,twenty-three anonymous feedbacks were received after the second round,two consensuses were reached at the third round,four were decided not to be recommended in the guideline,two were merged（nine for strong recommendations and nine for weak recommendations）,with the consensus rate of 75% and enthusiasm indexes of 100%.The conformity index of each recommendation was quite different ranged from 0 to 50.44%.CONCLUSIONThis article introduces and summarizes the methods and process of the three-round Delphi method to develop the recommendations,and provides methodological support for the development of the recommendations.
