基于Time-lapse技术评估痰湿型PCOS不孕患者早期胚胎动力学特点 被引量:4

Evaluation of early embryo dynamics parameters in PCOS infertility patients with phlegm damp type by Time-lapse

摘要 目的:利用Time-lapse技术评估痰湿型多囊卵巢综合征(PCOS)不孕患者早期胚胎动力学的特点。方法:91例常规长方案ICSI-ET治疗PCOS不孕患者,其中痰湿型45例(195个2PN胚胎),非痰湿型46例(240个2PN胚胎),同时选择月经周期规律的正常女性50例(230个2PN胚胎)为对照组。所有胚胎置于Time-lapse培养系统中培养5~7d。观察基线特征,胚胎早期动力学参数及获卵数、MII卵子数、受精率、种植率、妊娠率和流产率在3组间的差异。结果:与对照组比较,痰湿型和非痰湿型PCOS组t2、t3、t7、t8延长(P<0.05);与非痰湿型PCOS和对照组比较,痰湿型PCOS患者t4、t7延长(P<0.05);3组间cc2,cc3和s2差异无统计学意义;痰湿型和非痰湿型PCOS组的获卵数大于对照组(P<0.05);与非痰湿型PCOS和对照组比较,痰湿型PCOS组的种植率和妊娠率低,流产率高,但差异无统计学意义。结论:PCOS患者的t2、t3、t7和t8延长,痰湿型PCOS患者t4和t7延长,提示痰湿型PCOS患者的早期胚胎动力学参数存在异常。 Objective: To evaluate the characteristics of early embryonic kinetics parameters and the influence on pregnancy outcome in patients with PCOS by Time-lapse. Method: A total of 91 PCOS infertility patients accepted conventional long protocol down regulation in intracytoplasmic sperm injection and embryo transfer(ICSI-ET) were divided into three groups prospectively, phlegm-damp type of PCOS group, non-phlegm damp type of PCOS group, and control group. Baseline characteristics, the dynamic parameters of embryo quality, the number of retrieved oocytes and oocytes MII, fertilization rates, implantation rates, pregnancy rate, miscarriage rates were recorded. Result: Embryo from PCOS at t2, t3, t7 and t8 were significantly delayed compared with control(P〈0.05), Embryo from phlegm damp type PCOS at t4 and t7 were significantly delayed compared with control and non phlegm damp type PCOS(P〈0.05). There was no statistical difference in cc2, cc3 and s2, the number of retrieved oocytes were higher in PCOS than control(P〈0.05). There were lower implantation rate and pregnancy rate, and higher miscarry rate in phlegm damp PCOS group. Conclusion: Embryo from PCOS at t2, t3, t7 and t8 were significantly delayed, and t4 and t7 were significantly delayed in phlegm damp type PCOS infertility.
