摘要
目的探讨肥胖儿童载脂蛋白A5(Apo A5)的表达水平,并分析其与脂联素、空腹胰岛素的相关性。方法选择2014年1月至2015年12月收治的30例肥胖儿童,按照身高标准体重法分为轻、中、重度肥胖,同时选择30例健康体检者作为对照组,采集身高、体重、血脂、血糖等临床资料,同时采用酶联免疫吸附法检测血清Apo A5、脂联素水平,放射免疫分析法检测空腹胰岛素(FINS)水平,计算稳态模型评估的胰岛素抵抗指数(HOMA-IR)。结果肥胖儿童血清Apo A5表达水平明显低于对照组,随着肥胖程度的增加而降低(P<0.05);中、重度组甘油三酯(TG)、HOMA-IR、FINS、脂联素明显高于对照组、轻度组(P<0.05),但对照组与轻度组之间无显著差异(P>0.05);Apo A5与BMI、TG、HOMA-IR、FINS、脂联素均呈显著负相关(P<0.05);多元线性回归显示,TG、FINS、脂联素、HOMA-IR是儿童BMI的危险因素,Apo A5则是保护因素(P<0.05)。结论肥胖儿童血清Apo A5表达水平降低,与脂联素、FINS呈明显负相关,加强Apo A5干预,可能有助于儿童肥胖的治疗。
Objective To explore the expression of Apo A5 in obese children,and to analyze its relationship with adiponectin and fasting insulin( FINS). Methods 30 cases of obese children during January,2014 to December,2015 and 30 healthy controls in this hospital were selected for this study,all obese children were divided into light,medium and severe obesity according to their height and standard weight. The clinical data such as height,weight,blood lipids and blood glucose were collected,enzyme-linked immunosorbent assay( ELISA) was used to detect serum Apo A5,adiponectin,and radiation immunity analysis( RIA) was used to detect the level of fast insulin( FINS),homeostasis model assessment of insulin resistance( HOMA IR) was computed. Results The expression of serum Apo A5 in obese children was significantly lower than that of control group,and it decreased with the increase of obesity( P〈0.05). Levels of TG,HOMA IR,FINS and adiponectin in medium and severe obesity groups were significantly higher than those of control group and mild obesity group( P〈0.05),but the difference was not significant between control group and mild obesity group( P〉0.05). The expression of serum Apo A5 was negatively correlated with BMI,TG,FINS,adiponectin and HOMA-IR( P〈0.05). Multiple linear regression analysis showed that TG,FINS,adiponectin and HOMA-IR were risk factors for BMI of children,and Apo A5 was a protective factor( P〈0.05). Conclusion Obese children had low expression level of serum Apo A5,and it is negatively correlated with adiponectin and FINS. The strengthening of Apo A5 intervention may be helpful in the treatment of childhood obesity.
