摘要
[目的]分析中度活动度溃疡性结肠炎(UC)合并贫血患者蛋白质和脂代谢的变化及其临床意义,评估其营养状态。[方法]回顾性收集147例符合入选标准的中度活动度UC患者的蛋白质和脂代谢指标;比较合并贫血组患者(41例,占27.55%)与UC非贫血组患者(106例,占72.45%)血红蛋白(Hb),红细胞平均体积(MCV),红细胞平均血红蛋白浓度(MCHC),血清铁(Fe),铁蛋白、转铁蛋白、前蛋白(PA),总蛋白(TP),白蛋白(ALB),球蛋白(GLB),白蛋白球蛋白比值(A/G),总胆固醇(TC),三酰甘油(TG),高密度脂蛋白胆固醇(HDL-Ch),低密度脂蛋白胆固醇(LDL-Ch),总胆固醇与高密度脂蛋白胆固醇比值(T/HDL-Ch),脂蛋白a[LP(a)],载脂蛋白A1(ApoA1),载脂蛋白B(ApoB),ApoA1/B等指标的变化,并分析2组各指标异常的发生率的差异。[结果]合并贫血组Hb、TP、ALB、A/G、TC、HDL-Ch、LDL-Ch、ApoA1、ApoB指标明显低于非贫血组,差异有统计学意义(P<0.05);合并贫血组患者易发生TP、ALB、TC、A/G、HDL-Ch、LDL-Ch、ApoA1、ApoB的降低,与非贫血组比较差异有统计学意义(P<0.05)。[结论]中度活动度UC合并贫血患者营养处于负平衡状态,且更易出现多种蛋白质和脂代谢指标(TP、ALB、A/G、TC、HDL-Ch、LDL-Ch、ApoA1、ApoB)的降低。
[Objective]To analyze the changes of protein and lipid metabolism in moderate active ulcera- tive eolitis（UC）patients combined with anemia and its clinical significance,and to evaluate their nutritional status. [Methods]A retrospective study was performed in 147 cases of hospitalized moderate active UC pa- tients during June 2014 to June 2017. Descriptive data,disease related data and biochemical data were ob- tained from the medical records. The above indexes between the patients combined with anemic and without anemia were compared. [Results]Anemia occurred in 27.55% of all the patients. The levels of Hb,TP, ALB,A / G, TC, HDL-Ch, LDL-Ch, ApoA1 and ApoB were significantly lower in the patients combined with anemia than those without anemia（P-0.05）. The incidence of TP, ALB, A/G, HDL-Ch, LDL-Ch, ApoA1 ,ApoB were more likely to decrease in the patients combined with anemia（P〈0.05）. [Conclusions] The nutrition of the moderate activity UC patients that combined with anemia is in a negative balance state, and it is easier to show the abnormal decrease of many proteins and lipid metabolism indexes.
