摘要
目的：讨论保守治疗方法对老年人桡骨远端伸直型骨折的疗效研究。方法：选取2014年1月1日—2016年12月31日的老年桡骨远端骨折患者98例做为研究对象,完善处置前准备后以1%利多卡因行局部麻醉,麻醉起效后予适当牵引,根据骨折具体情况行手法复位,触诊确定复位效果后石膏外固定于掌屈尺偏位,处置后即刻复查DR确认复位情况,处置后3、10、24 d复查DR观察骨折断端对位情况并检查石膏松紧度。结果：所有患者均未进行手术,均获得骨性愈合,平均愈合时间（5.3±4.2）周,84例完成随访,末次随访时Green-O＇Brien腕关节评分优47例,良31例,可5例,差1例;患腕功能对日常生活均无严重影响;VAS评分平均为（2.1±1.3）分;治疗效果满意度100%。结论：老年桡骨远端骨折患者的治疗可尽量选择保守治疗,绝大多数可获得较好恢复;应考虑制定包括年龄因素在内的桡骨远端骨折分型。
Objective ：To study the curative effect of conservative treatment on distal radius of in old patients. Methods ：Selected 98 old patients of distal radius fracture continuously from January 1,2014 to December 31,2016. After preparation,each patient was anesthetized by 1% lidocaine locally. After appropriate traction,we took reduction to the fractures according to the specific situation. Then palpated to determine the effect of reduction. Fixed the fracture by plaster at the position of palm bow and ulnar deviation. Took DR to confirm the position of the fracture after reduction immediately. Also had the examination of DR at the 3 th,10 th,and 24 th days. At the same time checked the fitness of plaster. Results ：All patients did not undergo surgery and got osseous healing,with average time of（5.3±4.2）weeks. 84 people completed the follow-up at last. At the last time of follow-up 47 cases got the score of excellent by O＇brien wrist score,31 cases of good,5 cases of fair,1 case of poor. The function of all wrists can meet the need of daily life and the VAS score was（2.1±1.3）points. All patients were satisfied with the therapeutic effect. Conclusion ：Conservative treatment can cure most old patients of distal radius fractures and majority of the patients can get better recovery. Meanwhile the classification of distal radius fractures should take age as important factor.
