摘要
目的 探讨飞秒激光联合个体化准分子激光角膜切削术在近视患者中应用效果.方法 497例近视患者按照手术方式分组,对照组248例给予传统准分子激光原位角膜镶磨术治疗,观察组249例给予飞秒激光辅助个体化准分子激光角膜切削术治疗.观察比较两组临床治疗效果及手术前后各像差(彗差、高阶像差、球差)变化情况,并统计两组并发症发生情况及生活质量(QOL)评分.结果 观察组临床治愈率为97.99%(244/249),对照组为97.18%(241/248),组间比较差异未见统计学意义(P〉0.05);术后3个月两组彗差、高阶像差及球差均有所增加,但观察组增加倍数小于对照组,差异有统计学意义(P〈0.05);观察组并发症发生率为0.40%(1/249),低于对照组的3.63%(9/248),差异有统计学意义(P〈0.05);术后1年观察组QOL评分高于对照组,差异有统计学意义(P〈0.05).结论 飞秒激光联合个体化准分子激光角膜切削术可提高近视患者视力及生活质量,降低并发症发生率,且彗差、高阶像差、球差增加均较少.
Objective To investigate the application effect of femtosecond laser combined with individualized photorefractive keratotectomy on myopia patients. Methods A total of 497 patients with myopia were selected and were grouped according to treatment protocols. The control group (248 cases) were given conventional laser in situ keratomileusis, and observation group (249 cases) were given indi-vidualized photorefractive keratoteetomy assisted by femtosecond laser. The clinical treatment effect and every aberration (coma, high order aberration and spherical aberration) between two groups were com-pared before and after operation. The incidence of complication and the quality of life (QOL) score of two groups were recorded. Results There was an insignificant difference in clinical cure rate between observation group [97.99% (244/249) ] and control group [97.18% (241/248) ], P 〉0.05. The co-ma, high order aberration and spherical aberration were increased in the two groups 3 months after opera-tion, but the increasing rate of observation group was less than that of control group (P 〈 0.05). The in-cidence of complication in observation group [ 0.40% (1/249) ] was lower than that in control group [ 3.63 % (9/248) ], P 〈 0. 05. The QOL score of observation group was higher than that of control group 1 year after operation (P 〈 0. 05). Conclusions Femtosecond laser combined with individualized pho-torefractive keratotectomy can improve the QOL of patients with myopia, reduce the incidence of compli-cations, by which the coma, high order aberration and spherical aberration were increased less.
