益肾逐瘀法治疗强直少动型帕金森病临床观察 被引量:3

Clinical Observation of Parkinson's Disease of Rigidity and Hypoactivity Pattern Treated by the Method of Benefiting Kidney and Dispelling Blood Stasis

摘要 目的:观察益肾逐瘀法对强直少动型帕金森病(parkinson's disease,PD)的临床疗效。方法:选取2014年12月至2016年1月就诊于北京中医药大学东直门医院脑病科门诊的强直少动型帕金森病患者9例,均为右利手,将9例患者设为PD组。正常对照组为无神经、精神系统疾病及其他重大疾病的志愿者,均为右利手。PD组给予培元解痉汤加减治疗,正常对照组不作药物干预,比较不同观察时点PD相关量表分值。采集正常对照组及PD组治疗前后的f MRI数据,分析各组间不同脑区之间的FC。结果:PD组入组28 d时中医证候量化分级表评分较疗前显著下降(P<0.05),治疗后中医证候量化分级表、NMSS、PDQ-39评分及入组28 d时UPDRSⅢ评分较疗前均有改善的趋势,差异无统计学意义(P>0.05)。治疗前后对比形成的脑网络中杏仁核是一个重要节点,PD组治疗前右侧杏仁核与右侧壳核的FC大于对照组,而治疗后这两个区域的FC较治疗前减弱。结论:益肾逐瘀法对强直少动型PD具有一定的临床疗效,可能通过调整紊乱的脑功能连接而发挥治疗作用。 Objective: To observe the clinical effects of the method of benefiting kidney and dispelling blood stasis on Parkinson’ s disease( PD) of rigidity and hypoactivity pattern. Methods: Totally 9 PD patients of rigidity and hypoactivity pattern treated in the Dongzhimen Hospital between December 2014 and January 2016 were selected as PD group. Volunteers without nervous,mental or other serious disease were as normal control group. PD group were given modified Spasm-Relieving Decoction for the treatment while normal control group were not given any intervention. The scores of PD scales in different observation time were compared.The f MRI in both groups before and after treatment were collected,and the FC of different brain regions between each group was compared. Results: On the 28 d after grouping of PD group,the scores of TCM syndrome quantification grading scale declined markedly( P 〈 0. 05). After treatment,the TCM syndrome quantification grading scale,NMSS,PDQ-39 and score of UPDRSⅢ on the 28 d after grouping improved; the difference had no statistical significance( P 〉 0. 05). The amygdale formed in the cerebral network was a key point after treatment. The amygdales and the FC of amygdale in right side in PD group were greater than those in control group,and the FC declined in these two areas after treatment. Conclusion: The method of benefiting kidney and dispelling blood stasis has certain clinical effects on PD of rigidity and hypoactivity pattern,which exerts its effects by regulating the disordered cerebral functional concectivity.
