单核细胞/高密度脂蛋白胆固醇比值在Ⅱ型糖尿病患者并发肾病中的临床意义 被引量:4

摘要 目的分析单核细胞/高密度脂蛋白胆固醇比值在糖尿病肾病患者中的临床意义。方法收集2015年1月‐2016年11月该院诊治的Ⅱ型糖尿病患者87例为研究对象,依据24-hour尿白蛋白排泄量(UAE)将其分为正常组(22例,<30 mg)、微量组(27例,30~300 mg)和大量组(38例,>300 mg)。统计学分析3组患者临床病理学参数的差异及单核细胞/高密度脂蛋白胆固醇比值与临床病理参数之间的相关性。结果 3组患者血清肌酐水平、24 h尿白蛋白排泄量和单核细胞/高密度脂蛋白胆固醇比值比较差异均有统计学意义(P<0.01);87例Ⅱ型糖尿病患者单核细胞/高密度脂蛋白胆固醇比值与24 h尿白蛋白排泄量呈正相关(r=0.397,P<0.0001);87例Ⅱ型糖尿病患者单核细胞/高密度脂蛋白胆固醇比值与肾小球滤过率呈负相关(r=-0.250,P=0.0196)。结论单核细胞/高密度脂蛋白胆固醇比值可用于评估糖尿病早期肾损伤,对于Ⅱ型糖尿病合并肾病患者具有预后价值。 [Objective] To investigate the clinical significance of monocyte/high density lipoprotein cholesterol ratio (MHR) in type Ⅱ diabetes mellitus patients with nephropathy. [Methods] Eighty-seven type II diabetes mellitus patients hospitalized from January 2015 to November 2016 were included in this study. According to the amount of urinary albumin excretion during 24 hour (24-hour UAE), patients were divided into three groups: normoalbuminuria group (22 patients, 〈30 mg), microalbuminuria group (27 patients, 30-300mg), macroalbuminuria group (38 patients, 〉300 mg). Differences of clinicopathological parameters in three groups and the relationships between clinicopathological parameters and MHR were statistically analyzed. [Results] Serum creatinine (P =0.006), 24-hour UAE (P 〈0.001), and MHR (P =0.003) were all significantly different among the three groups. In 87 patients of type II diabetes mellitus with nephropathy MHR was positively correlated with 24-hour UAE (r = 0.397, P 〈0.0001) and negatively correlated with glomerular filtration rate (GFR) (r =-0.250, P = 0.0196). [Conclusion] MHR can be used to evaluate the renal iniurv and is a useful early prognostic marker in diabetic nephropathy patients.
