摘要
[目的]系统评价延续性护理对改善乳腺癌病人焦虑抑郁症状、提高生活质量的效果。[方法]计算机检索中国知网(CNKI)、万方、维普、中国生物医学数据库(CBM)、谷歌学术、PubMed、Cochrane Library、Web of Science等中英文数据库。检索时间限定为从建库至2017年3月。由2名研究者按照文献纳入和排除标准独立筛选文献,提取资料并评价文献质量后,采用RevMan 5.3进行Meta分析。[结果]本研究共纳入9个随机对照试验,共1 318例乳腺癌病人。所纳入的文献质量等级均为B级。Meta分析结果显示:延续性护理可以改善乳腺癌病人的焦虑[MD=-3.19,95%CI(-4.90,-1.48),P<0.000 3]和生活质量[SMD=-0.70,95%CI(-1.03,-0.37),P<0.000 1];描述性结果显示:延续性护理乳腺癌病人生活质量子条目与常规护理组相比,差异有统计学意义(均P<0.01)。[结论]延续性护理可以改善乳腺癌病人的焦虑症状,提高病人生活质量,但受纳入研究质量限制,还需要更多高质量研究予以验证。
Objective：To evaluate the effect of continuous nursing on improving anxiety,depression and quality of life of patients with breast cancer. Methods： Database including CNKI, Wanfang Data, VIP, CBM, Google Scholar, PubMed,Cochrane Library and Web of science were electronically searched from inception to March 2017. According to inclusion and exclusion criteria of literatures, two reviewers independently screened literature, extracted data and evaluated the quality. RevMan 5. 3 software was used for Meta-analysis. Results： A total of 9 RCTs involving 1 318 patients with breast cancer were included. The quality level of the literatures were all grade B. Meta-analysis indicated that continuous nursing could improve the anxiety[ME）：- 3.19,95 % CI（-4. 90,-1.48）, P = 0. 000 3] and quality of lifeVSMD=0.70,95% CI（-1.03,-0.37）,P〈0.000 1]of patients with breast cancer. Descriptive results showed that there were significant differences in the subitems of the quality of life between breast cancer patients in routine nursing group and continuous nursing group（P〈0.01）. Conclusion：Continuous nursing can improve the symptoms of anxiety in the patients with breast cancer and improve the quality of life of the patients. The researon was limited by the quality of induded researches so it needs to be verified by more high quality of researches.
