创伤性颅脑损伤患者中降钙素原、CRP及D-二聚体的临床检测意义 被引量:8

摘要 目的:研究创伤性颅脑损伤(traumatic brain injury,TBI)患者中降钙素原(procalcitonin,PCT),CRP,D-二聚体水平与患者病情程度及临床预后中的价值。方法:分析2014年1月至2016年12月接受治疗的250例TBI患者,采用格拉斯哥昏迷评分法(Glasgow Coma Scale,GCS)评估病情,电化学发光法及免疫散射比浊法检测患者血清中PCT,CRP和D-二聚体水平,分析其与患者病情的相关性,Cox回归模型探讨与患者死亡的关系。结果:TBI患者中PCT,CRP,D-二聚体水平随着病变的严重程度增加明显升高,差异具有统计学意义(P<0.05);存活组GCS评分显著高于死亡组(P<0.01),而PCT,CRP及D-二聚体水平均显著低于死亡组(P<0.01);多因素分析提示GCS评分,PCT,CR P以及D-二聚体是T B I患者死亡的独立因素;PCT,CR P,D-二聚体水平与患者入院时GCS评分存在负相关,相关系数分别为–0.23,–0.32,–0.42(均P<0.05)。结论:PCT,CRP,D-二聚体水平与TBI患者的损伤程度及预后具有明显相关性。 Objective: To analyze the effects of procalcitonin(PCT), CRP, D-dimer on the severity and clinical prognosis of patients with traumatic brain injury(TBI). Methods: A total of 250 TBI patients were recruited from January 2014 to December 2016, and the disease severity was assessed by Glasgow Coma Scale(GCS) score. Levels of PCT, CRP and D-dimer were detected and their co-relationship with patient’s condition and death rate were investigated, and the Cox review model was used to explore the relationship with the patient’s death. Results: Level of PCT, CRP and D-dimer in TBI patients increased significantly with the severity of the lesion; the level of GCS score of patients in the survival group elevated significantly when compared with that in the death group, whereas the levels of PCT, CRP and D-dimer in the survival group decreased significantly. Multivariate analysis indicated that PCT, CRP, D-dimer and GCS score were independent risk factors for death in TBI patients. The levels of PCT, CRP and D-dimer were positive with GCS(r=-0.23,-0.32,-0.42; P〈0.05). Conclusion: Level of PCT, CRP and D-dimer closely associated with the severity and prognosis of TBI patients.
