摘要
目的比较MRI纵向弛豫时间定量(T1 mapping)、横向弛豫时间定量(T2 mapping)、血氧水平依赖(BOLD)成像对大鼠肝纤维化的诊断效能以及分期评估价值。方法对53只大鼠肝纤维化模型进行MRI T1 mapping、T2 mapping、BOLD成像,测量肝实质T1值(ms)、T2值(ms)和R2*值(Hz)与病理结果对照,以病理肝纤维化≥S1期为阳性标准,运用受试者工作特性曲线(ROC)比较三者对肝纤维化的诊断效能,同时比较三者与肝纤维化各期的相关性。结果病理学肝纤维化S(0-4)期大鼠肝脏的T1值、T2值和R2*值差别均具有统计学意义,三者的曲线下面积、灵敏度和特异度均为T2值〉T1值〉R2*值。T1值与肝纤维化病理学分期呈正相关,S0和肝纤维化各期、轻度肝纤维化(S(1-2)期)和重度肝纤维化(S(3-4)期)之间比较差异均具有统计学意义,而S1和S2、S3和S4之间比较差异无统计学意义。T2值和R2*值与肝纤维化病理学分期相关性分析均无统计学意义。结论T1 mapping、T2 mapping对大鼠肝纤维化的诊断效能较BOLD高,而T1 mapping在评价大鼠肝纤维化程度的价值较BOLD和T2 mapping高。
Objective The present study was designed to compare the diagnostic efficacy and evaluating the stage of rat's liver fibrosis of T1 mapping,T2 mapping and blood oxygenation level dependent imaging( BOLD). Methods 53 rat models of liver fibrosis were imaged with liver MRI scan including T1 mapping、T2 mapping、BOLD imaging. T1( ms),T2( ms) and R2*( Hz) values measured in liver parenchyma comparing with liver histological specimens was to analyze correlation between pathological stages of liver fibrosis( S(0-4)) and the MRI parameter. We set fibrosis degree greater than/or equal S1 as positive criteria and used receiver operating characteristic( ROC) curve to evaluate diagnostic efficacy of T1,T2 and R2*values. Results T1,T2 and R2*values were statistically different in five pathological stages. Area under the curve( AUC),sensibility and specificity showed T2 value T1 value R2*value. T1 value was positively correlated with the pathological stages. S0 verse each stage of liver fibrosis displayed significant difference,same result in Mild liver fibrosis(S(1-2)) verse severe liver fibrosis(S(3-4)). There was no significant difference in S1 verse S2,S3 verse S4,similar negative result in T2 and R2*values among each stage of liver fibrosis. Conclusion T1 mapping and T2 mapping manifested higher diagnostic efficacy than BOLD in liver fibrosis of rats. T1 mapping has the most important value for evaluating the severity of liver fibrosis in rats,which had higher accuracy and reproducibility than BOLD and T2 mapping.
