摘要
目的探讨T1mapping对大鼠肝纤维化分期的诊断价值以及与血清肝纤维化标志物的相互关系。方法对34只大鼠肝纤维化模型进行MRI T1mapping成像和血清肝纤维化标志物检测,测量肝实质T1值(ms)和检测血清标志物透明质酸(HA)、Ⅲ型前胶原(PIIIP)、Ⅳ型胶原(C-IV)和层粘连蛋白(LN)的含量,根据肝纤维化病理学分期将大鼠划分为无肝纤维化组(S0期)、轻度肝纤维化组(S1~2期)和重度肝纤维化组(S3~4期),运用受试者工作特性(ROC)曲线评价T1值和血清标志物在肝纤维化分期的诊断价值,并进行两者之间的相关性分析。结果在判断有无肝纤维化和区分轻、重度肝纤维化中,ROC曲线下面积大小依次为T1+HA+PIIIP(0. 85)> T1(0. 72)> PIIIP(0. 68)> HA(0. 61)> C-IV(0. 58)> LN(0. 46)和T1+HA+PIIIP(0. 93)> T1(0. 87)> C-IV(0. 75)> PIIIP(0. 70)> HA(0. 67)> LN(0. 57)。T1值和血清标志物指标与肝纤维化程度呈正相关,T1值与HA具有一定相关性(r=0. 412,P <0. 05),与C-IV、PCIII和LN无相关性(P> 0. 05)。结论磁共振T1mapping在判断有无肝纤维化和区分轻、重度肝纤维化中具有较高的诊断价值,与血清标志物相结合则诊断价值更高。
Objective The present study was designed to investigate the diagnostic value of T1 mapping in the stage of liver fibrosis in rat and the relationship between T1 mapping and serum fibrosis markers. Methods 34 rats underwent MRI T1 mapping and serum markers,T1value( ms) and serum fibrosis markers include hyaluronic acid( HA),procollagen typeⅢ peptide( PIIIP),type IV collagen( C-IV) and laminin( LN) levels( μg/L) were measured. The rats were divided into control group( S0),mild group( S1-2) and severe group( S3-4) according to the pathological stages of liver fibrosis.Receiver operating characteristic( ROC) curve was used to evaluate the diagnosis performance of T1 value and serum markers in liver fibrosis staging,while relationship between them was analyzed. Results The area under the curve( AUC) of the liver fibrosis severity estimated by T1 value and serum marker levels were in the order of T1+ HA + PIIIP( 0. 85) > T1( 0. 72) > PIIIP( 0. 68) > HA( 0. 61) > C-IV( 0. 58) > LN( 0. 46) and T1+ HA + PIIIP( 0. 93) > T1( 0. 87) > C-IV( 0. 75) > PIIIP( 0. 70) > HA( 0. 67) > LN( 0. 57). There was positive relationship between T1 and serum markers. A significant correlation was found between T1 value and HA( r = 0. 412,P < 0. 05),but had no correlation between T1 value and C-IV,PIIIP,LN( P > 0. 05). Conclusion T1 mapping had a high diagnostic value in estimating liver fibrosis,while the combination of T1 mapping and serum markers had a higher diagnostic value.
