摘要
目的了解慢性肺源性心脏病(chronic pulmonary heart disease,CPHD)患者的生存质量,并分析其与主要心肺功能指标的相关性。方法收集2014年1月—2016年12月简阳市人民医院呼吸内科收治急性加重期CPHD患者168例;采用CPHD生存质量测定量表(QLICD-CPHD)对患者的生存质量进行测评;查阅病历资料,收集患者的1秒用力呼气容积(FEV1)及其占正常预计值的百分比(FEV1%)、1秒用力呼气容积/最大肺活量(FEV1/FVC)、肺动脉压(PAP)、右心室射血分数(EFRV)、血氧分压(PaO_2)、二氧化碳分压(PaCO_2)等主要心肺功能指标;分析生存质量总分及各领域得分与主要心肺功能指标的相关关系,采用多重线性回归模型分析影响患者生存质量相关因素。结果 CPHD患者生存质量标准化总分为58.8±12.4,社会功能领域(SOD)得分最高(63.1±15.4),生理功能领域(PHD)得分最低(54.6±13.2)。相关性分析显示,SOD得分与FEV1、FEV1%、FEV1/FVC、PaCO_2呈正相关关系(P<0.05);PAP与PHD得分、慢性肺源性心脏病疾病特异模块(SPD)得分及QLICD-CPHD量表总分呈负相关关系(P<0.05);EFRV与PHD得分、SPD得分呈正相关关系(P<0.05);PaCO_2与PHD得分、QLICD-GM模块心理功能领域(PSD)得分、SPD得分及QLICD-CPHD量表总分呈负相关关系(P<0.05)。多重线性回归分析显示:对CPHD患者QLICD-CPHD量表总分有影响的因素(按影响程度大小排序)有肺动脉压(PAP)和二氧化碳分压(PaCO_2);对CPHD疾病特异模块(SPD)得分有影响的因素(按影响程度大小排序)有肺动脉压(PAP)、右心室射血分数(EFRV)和二氧化碳分压(PaCO_2)。结论 CPHD患者生存质量评价中,生理功能(PHD)得分最低;心肺功能指标中,肺动脉压(PAP)、右心室射血分数(EFRV)和二氧化碳分压(PaCO_2)与患者生存质量的相关性较高。
[Objective] To understand the quality of life in chronic pulmonary heart disease(CPHD)patients,investigate the correlation between quality of life and values of cardiopulmonary function.[Methods]168 CPHD patients in acute exacerbation period,who were treated in the Department of respiratory of the People's Hospital of Jianyang City from January 2014 to December 2016,were enrolled in this study. The quality of life instrument for chronic diseases-CPHD(QLICD-CPHD)survey was used to evaluate the quality of life(QOL)in the patients. The values of patients' cardiopulmonary function consisted of forced expiratory volume in one second(FEV1),forced expiratory volume accounted for 1 percent of the normal expected value(FEV1%),forced expiratory volume in one second/forced vital capacity(FEV1/FVC),pulmonary artery pressure(PAP),right ventricular ejection fraction(EFRV),oxygen partial pressure(PO2) and partial pressure of carbon dioxide(PaCO2) were collected from the medical records. The correlations between total score of QOL,fact score and values of cardiopulmonary function were analyzed,and the influencing factors were analyzed by the multiple linear regression model.[Results]The standardized score of QOL in CPHD patients was 58.8±12.4,while the score of social function domain(SOD) was the highest(63.1±15.4) and that of physiological function domain(PHD) was the lowest(54.6 ±13.2). Correlation analysis showed that the score of SOD was positively correlated with FEV1,FEV1%,FEV1/FVC and PaCO2(P0.05),there were the negative correlation between PAP and PHD score,SPD score,QLICD-CPHD score(P0.05),EFRV was positively correlated with PHD score and SPD score,and PaCO2 was negatively correlated with PSD score,SPD score and QLICD-CPHD score(P0.05). Multiple linear regression analysis showed that the influencing factors of QLICD-CPHD score in CPHD patients included PAP and PaCO2(sorted by the influence degree),while the influencing factors of SPD were PAP
