摘要
目的探究双侧锁定钢板内固定和单侧钢板加空心钉内固定修复胫骨平台骨折的临床治疗效果。方法 80例胫骨平台骨折患者,根据治疗方式的不同分为双侧锁定钢板内固定组和单侧钢板加空心钉内固定组,每组40例,观察并比较两组患者的骨折复位与愈合情况及Rasmussen膝关节功能评分。结果双侧锁定钢板内固定组住院时间、术后开始负重训练时间、骨折愈合时间、胫骨平台内翻角(TPA)、胫骨平台内侧后倾角(PA)均优于单侧钢板加空心钉内固定组,差异具有统计学意义(P<0.05)。双侧锁定钢板内固定组Rasmussen膝关节功能评分优、良、可、差分别为15、15、8、2例,单侧钢板加空心钉内固定组Rasmussen膝关节功能评分优、良、可、差分别为5、16、10、9例;双侧锁定钢板内固定组Rasmussen膝关节功能评分优于单侧钢板加空心钉内固定组,差异具有统计学意义(P<0.05)。结论双侧锁定钢板内固定修复胫骨平台骨折具有更好的治疗效果,可以在今后的胫骨平台骨折治疗中应用和推广。
Objective To investigate the clinical treatment effect of bilateral locking plate and unilateral steel and hollow nails internal fixation for the repair of tibial plateau fracture. Methods A total of 80 tibial plateau fracture patients were divided by different treatment methods into bilateral locking plate group and unilateral steel and hollow nails group, with 40 cases in each group. Observation and comparison were made on fracture reduction and healing and Rasmussen knee joint function score between the two groups. Results Bilateral locking plate group had better hospitalization time, weight training time after operation, fracture healing time, tibial plateau varus (TPA), posterior slope angle (PA) than unilateral steel and hollow nails group, and the difference was statistically significant (P〈0.05). Bilateral locking plate group had excellent, good, moderate and poor of Rasmussen knee joint function score respectively as 15, 15、8 and 2 cases, which were respectively 5, 16, 10 and 9 cases in unilateral steel and hollow nails group. Bilateral locking plate group had better Rasmussen knee joint function score than unilateral steel and hollow nails group, and the difference was statistically significant (P〈0.05). Conclusion Bilateral locking plate internal fixation shows better treatment effect in the repair of tibial plateau fracture, and can be applied and promoted in treatment of tibial plateau fracture in the future.
