摘要
目的对比分析肱骨近端骨折切开复位锁定钢板内固定术与闭合复位Multiloc肱骨近端髓内钉系统内固定术的临床疗效。方法60例肱骨近端骨折患者,按随机数字表法分为对照组(40例)和研究组(20例)。对照组采用切开复位锁定钢板内固定术治疗,研究组采用闭合复位Multiloc肱骨近端髓内钉系统内固定术治疗。比较两组手术相关指标、骨折愈合时间和Constant肩关节评分、疼痛评分、并发症发生情况。结果研究组切口长度(4.72±1.23)cm、手术用时(77.41±10.39)min均短于对照组的(10.53±2.75)cm、(94.46±12.61)min,失血量(83.27±20.45)ml少于对照组的(128.88±22.53)ml,差异具有统计学意义(P<0.05)。两组骨折愈合时间与Constant肩关节评分比较,差异无统计学意义(P>0.05)。术后,研究组的疼痛评分(1.54±0.63)分低于对照组的(2.76±1.08)分,差异具有统计学意义(P<0.05)。两组的并发症发生率比较,差异无统计学意义(P>0.05)。结论与切开复位锁定钢板内固定术相比,闭合复位Multiloc肱骨近端髓内钉系统内固定术治疗肱骨近端骨折患者的效果显著,该术式操作简单、失血量少、创伤小,有利于患者术后早日康复。
Objective To compare the clinical efficacy of open reduction and internal fixation with locking plate and closed reduction and internal fixation with Multiloc proximal humeral nail system for proximal humeral fractures.Methods A total of 60 patients with proximal humeral fractures were divided into control group(40 cases)and research group(20 cases)according to random numerical table.The control group was treated with open reduction and internal fixation with locking plate,and the research group was treated with closed reduction and internal fixation with Multiloc proximal humeral nail system.The surgical related indicators,fracture healing time,Constant shoulder score,pain score,and occurrence of complications were compared between the two groups.Results The incision length(4.72±1.23)cm and hospitalization time(77.41±10.39)min of the research group were shorter than(10.53±2.75)cm and(94.46±12.61)min of the control group,and the blood loss(83.27±20.45)ml was less than(128.88±22.53)ml of the control group.All the difference was statistically significant(P<0.05).There was no statistically significant difference in fracture healing time and Constant shoulder score between the two groups(P>0.05).After surgery,the pain score(1.54±0.63)points of the research group was lower than(2.76±1.08)points of the control group,and the difference was statistically significant(P<0.05).There was no statistically significant difference in incidence of complications between the two groups(P>0.05).Conclusion Compared with open reduction and internal fixation with locking plate,closed reduction and internal fixation with Multiloc proximal humeral nail system has more significant effect on patients with proximal humeral fractures,with simple operation,less blood loss and less trauma,which is conducive to the early recovery of patients after surgery.
