摘要
目的构建科室多维度规培系统并评价其对住院医师规培效果。方法纳入2015年11月-2017年4月某院规培医师(系统构建前后分别38及41人),同期教学老师共17人。通过专科考核(客观题60分,病例分析40分,专业技能100分)、对住院医师(0~10分)和教师(明显提高、提高、无明显差别、降低)问卷调查的方法,对多维度规培系统的构建前后的培训效果进行评价。结果与传统培训模式相比,多维培训模式下规培医师病例分析(26.84±4.402 vs 33.37±3.923,P<0.01)、专业技能(77.24±5.069 vs81.41±5.128,P<0.01)均有提高。教师认为多维培训模式在培养主动性、临床思维、专业技能、职业素质方面较传统培训模式提高,分别是5(29.4%)、5(29.4%)、7(41.2%)、8(47.1%)。同时分别有8(47.1%)、9(52.9%)、7(41.2%)、7(41.2%)的教师认为明显提高。规培医师对多维规培模式的接受程度(8.02±0.885vs 7.2±0.985,P<0.01)和培训效果(8.06±0.843 vs 7.31±1.047,P<0.01)均较传统培训模式提高。结论多维度科室规培系统不仅能提高住院医师临床诊疗能力,同时可以提高医学素养。
Objectives To construct a multi-dimensional system for resident standardized training and evaluate the effect. Methods Thirty eight and forty one physicians before and after the system building were included from November 2015 to April 2017. At the same time, seventeen teachers were surveyed. The effect of the multi-disciplinary system were evaluated before and after construction by specialist examination (objective topic 60 points, cases analysis 40 points, professional skills 100 points), questionnaire from the resident (scores 0-10) andteachers (obviously improved, improved, no difference, decline). Results Compared to the traditional training mode, the scores of cases analysis (26. 84± 4. 402 vs 33.37±3. 923, P〈0. 01), professional skills (77.24 ± 5. 069 vs 81.41 ± 5. 128, P〈0. 01) were improved in multi-dimensional training mode. The ratio of teachers who considering that initiative cultivating, clinical thinking, professional skills and professional quality been elevated by multidimensional training mode were 5 (29.4%), 5 (29.4%), 7 (41.2%), 8 (47.1%) respectively, and 8 (47.1%), 9 (52.9%), 7 (41.2%), 7 (41.2%) obviously improved. The extent for accept for the new mode (8.02±0. 885 vs 7.2 ± 0. 985, P〈0. 01) and training effects (8.06± 0. 843 vs 7. 3± 1. 047, P〈0. 01) improved. Conclusions The multi-dimensional system could not only improve the clinical diagnosis and treatment capacity of residents, but also improve their medical professional quality.
