摘要
目的:探讨不同剂量国产普瑞巴林用于带状疱疹后神经痛(post-herpetic neuralgia,PHN)治疗的疗效与安全性。方法:45例诊断为PHN且符合纳入标准的患者经知情同意,采用随机数字表法分为小剂量组(n=15)、中剂量组(n=15)、大剂量组(n=15),分别给予225、300、450 mg/d国产普瑞巴林治疗4周。于治疗前,治疗1、2、3、4周观察患者疗效及不良反应。采用视觉模拟量表(visual analogue scale,VAS)、医院焦虑抑郁量表(hospital anxiety and depression,HAD)、医学结局研究(medical outcomes study,MOS)睡眠量表分别评估患者疼痛、焦虑与抑郁及睡眠状况。结果:3个剂量组患者治疗1、2、3、4周时VAS评分、MOS睡眠量表评分均较治疗前下降,差异有统计学意义(P<0.05);治疗4周时3组患者HAD评分较治疗前也有降低,差异有统计学意义(P<0.05);但3个剂量组间VAS评分、疼痛缓解疗效、MOS睡眠量表评分、HAD评分比较差异并无统计学意义(P>0.05)。不良反应以头晕、口干、嗜睡为主,多为一过性。3组间比较,头晕、口干、嗜睡各不良反应间差异有统计学意义(P<0.05),余不良反应间差异无统计学意义(P>0.05)。结论:不同剂量国产普瑞巴林均可减轻PHN患者疼痛,改善焦虑、抑郁、睡眠,其疗效不一定为剂量依赖性,而其不良反应为轻中度且能自行缓解。
Objective：To investigate the efficacy and safety of different doses of domestic pregabalin on post-herpetic neuralgia（PHN）.Methods：A total of 45 PHN patients who met the inclusion criteria were divided into three groups randomly,and different doses of domestic pregabalin were given for 4 weeks,respectively.The efficacy was evaluated by visual analogue scale（VAS）,hospital anxiety and depression scale（HAD）,Medical Outcomes Study（MOS）sleep scale and adverse reactions before treatment and 1 week,2 weeks,3 weeks and 4 weeks after treatment respectively.Results：After 1 week,2 weeks,3 weeks and 4 weeks of treatment respectively,the VAS scores and MOS sleep scores of three groups decreased significantly compared with that before treatment,and the differences were statistically significant（P〈0.05）.After 4 weeks,the HAD scores of three groups decreased significantly compared with that before treatment,and the differences were statistically significant（P〈0.05）.However,the differences in VAS scores,rates of pain relief,MOS sleep scores and HAD scores were not statistically significant in the same period（P〉0.05）.Finally,there were some transient and less severe adverse reactions among three groups,such as dizziness,dry mouth and somnolence,and the differences were statistically significant（P〈0.05）.Other adverse reactions were not statistically significant（P〉0.05）.Conclusion：For PHN patients,different doses of domestic pregabalin can alleviate the pain,and improve their anxiety,depression and sleep.The efficacy of domestic pregabalin is not necessarily dose-dependent,and its adverse reactions are mild,moderate and can be relieved by itself.
