富血小板纤维蛋白联合自体软骨颗粒修复兔耳软骨缺损 被引量:3

Experimental Study of Auricular Cartilage Defects Repair on Rabbits by PRF Combined with Autologous Cartilage Granules

摘要 目的:比较不同条件下兔耳软骨缺损区的修复情况,了解富血小板纤维蛋白(platelet-rich fibrin,PRF)联合自体软骨颗粒修复软骨缺损的有效性和可行性,为临床中软骨移植术后供区缺损修复提供参考。方法:选取30只新西兰大白兔,于双耳对称处设计1.0cm×0.6cm的全层软骨缺损,随机分为空白对照组、颗粒软骨组、PRF组、PRF+颗粒软骨组、软骨块回置组,各组予以对应处理。分别于4周,8周和12周后处死,观察各组缺损大体情况并行HE染色分析软骨修复情况,进行组织学修复评分,所得数值进行统计学分析。结果:大体标本和组织学观察可见:空白对照组几乎无软骨再生,为纤维组织覆盖修复;颗粒软骨组、PRF组软骨缺损边缘及移植的颗粒软骨周围可见少量新生软骨细胞,新生软骨较正常软骨薄且不规则;PRF+颗粒软骨组修复效果优于上述3组,缺损区为新生软骨覆盖,修复效果接近软骨块回置组。软骨缺损组织学修复评分结果显示:PRF+颗粒软骨组修复效果显著优于空白对照组(P<0.01),颗粒软骨组、PRF组软骨缺损修复效果优于空白对照组(P<0.05),颗粒软骨组、PRF组组间修复效果比较差异无统计学意义(P>0.05)。结论:PRF联合自体软骨颗粒能够有效修复及缩小软骨缺损面积,PRF或颗粒软骨单独置入修复效果也优于软骨缺损自行修复。 Objective To understand the effectiveness and feasibility of platelet-rich fibrin(PRF)combined with autologous cartilage granules to repair rabbits&#39; auricular cartilage defects and provide reference for the repair of cartilage defect in clinic.Methods Made symmetrical 1.0cm×0.6cm full-thickness cartilage defect on the 30 cases rabbit auricles by trauma as animal model,randomly divided into 5 groups:the black control group,PRF group,diced cartilage group,PRF+diced cartilage group and cartilage block replantation group,corresponded treatment according to grouping.The rabbits were sacrificed at 4 weeks,8 weeks and 12 weeks,check the cartilage defect repair situation by the general observation and histological observation(HE)staining.Results The general observation results and the histological result show that the black control group was almost no new cartilage formation,it was repaired by fiber texture.In the diced cartilage group or PRF group,there were some new cartilage tissues on the edge of the defect area and around the diced artilage,but the new cartilage tissue was differernt from the normal cartilage.In PRF+diced cartilage group the gross observation see that the cartilage defect area was covered by new cartilage tissue,the repair effect is close to the cartilage block replantation group.The score of histological repair of cartilage defect showed that there was significant difference between the black control group and PRF+diced cartilage group(P〈0.01),there was no significant diference between the diced cartilage group and PRF group(P〉0.05),but there was significant difference among the rest groups(P〈0.05).Conclusion Autologous cartilage granules composite PRF as a kind of material can effectively repair rabbit's ear full-thickness cartilage defects.The repair effect of PRF or diced cartilage implant alone is superior to that of cartilage defects.
