摘要
目的探讨多层螺旋CT(MSCT)下病灶内支气管动脉形态特征在肺部良恶性疾病中的诊断价值。方法选择94例病灶累及肺段及以上的肺部疾病患者,分为对照组(肺部良性疾病患者)28例和肺癌组(肺癌患者)66例。由两位有5年以上工作经验的放射科医师对支气管动脉的特征进行评价,将原始图像进行最大密度投影、容积再现和多平面重组,比较对照组与肺癌组患者支气管动脉形态特征的差异。结果 94例患者中,48例患者可见病灶内支气管动脉,其中对照组患者12例,肺癌组患者36例。对照组病灶内支气管动脉情况如下:血管湖患者0例,迂曲患者9例,成团患者1例,中断患者2例;肺癌组病灶内支气管动脉情况如下:血管湖患者10例,成团患者5例,迂曲患者18例,中断患者3例。对照组患者成团和血管湖征象少于肺癌组。结论 MSCT下病灶内支气管动脉形态特征对肺部良恶性疾病具有一定的诊断价值。
Objective To investigate the diagnostic value of bronchial artery morphological features in the diagnosis of benign and malignant lung disease using multi-slice computed tomography(MSCT). Method 94 cases of lung disease patients with pulmonary involvement were included in the study, in which 28 cases(with benign pulmonary disease)were assigned as control group, while 66 cases(with lung cancer) were stratified as lung cancer group. Two radiologists with more than 5 years of working experience were responsible for the evaluation of bronchial artery characteristics, the original imaging were processed with the maximum intensity projection, volume rendering and multi planar reconstruction, and the difference between the characteristics of bronchial artery morphology in control group and lung cancer group were compared. Result Of the 94 patients, 48 patients were observed with bronchial arteries in the lesion, of which 12 were in the control group and 36 in the lung cancer group. In control group, the distribution of bronchial artery morphology was as following: no one was with venous lake, 9 were tortuous pattern, 1 was with artery mass, and there were 2 cases of bronchial artery interruption; in lung cancer group: there were 10 cases of venous lake, 5 cases of artery mass, and 18 cases were with tortuous pattern, and 3 patients were with artery interruption. The signs of venous lake were less in the control group compared with lung cancer group. Conclusion The morphological features of bronchial arteries in lung cancer lesions under MSCT are practical for the diagnosis and treatment of lung cancer.
