摘要
目的探讨64层螺旋CT在甲状腺结节良恶性中的诊断价值。方法选取垂杨柳医院医联体2015年12月至2016年12月收治的甲状腺结节患者102例,所有患者均行64层螺旋CT平扫及增强扫描,观察并统计纳入患者CT表现差异及病理学诊断结果;以病理学诊断为依据,计算64层螺旋CT诊断甲状腺良恶性疾病的准确度、特异度、灵敏度。结果 64层螺旋CT可清楚显示甲状腺结节的部位、直径、形态、边界、钙化及囊变情况,良恶性甲状腺结节在形态、边界、钙化方面,差异均具有统计学意义(P=0.000)。良性结节患者均匀强化(50.00%)、完整环形强化(41.07%)、瘤周强化残圈(0)、半岛样强化(3.57%)与恶性结节患者(4.35%、6.52%、45.65%、34.78%)比较,差异均有统计学意义(P=0.000)。64层螺旋CT扫描结果显示,甲状腺良性结节58例,恶性结节44例,特异度为92.86%,灵敏度为86.96%,准确度为90.20%。结论 64层螺旋CT诊断甲状腺良恶性疾病具有较高的准确度、特异度、灵敏度。
Objective To evaluate the diagnostic value of 64 slice spiral CT in benign and malignant thyroid nodules. Methods A total of 102 patients with thyroid nodules was selected in Beijing Chui Yang Liu Hospital Affiliated to Tsinghua University from December 2015 to December 2016, and all patients underwent 64 slice spiral CT plain scan and enhanced scan. Then the CT findings and pathological findings were observed and analyzed. Based on pathological diagnosis, the accuracy, specificity and sensitivity of 64 slice spiral CT diagnosis of benign and malignant thyroid diseases were calculated. Results The location, diameter, shape, boundary, calcification and cystic degeneration of thyroid nodules could be clearly shown by using 64 slice spiral CT. Differences were statistically significant in morphology,boundary and calcification between benign and malignant thyroid nodules(P=0.000). In patients with benign and malignant nodules, homogeneous enhancement(50.00% vs 4.35%), complete ring reinforcement(41.07% vs 6.52%), the residual circle(0 vs 45.65%) and peninsula samples(3.57% vs 34.78%) were statistically different(all P=0.000). 64 slice spiral CT scan results showed 58 cases of benign thyroid nodules and 44 cases of malignant nodules, with specificity of92.86%, sensitivity of 86.96% and accuracy of 90.20%. Conclusion 64 slice spiral CT shows relatively high accuracy,specificity and sensitivity for the diagnosis of benign and malignant thyroid nodules.
