摘要
目的探讨在甲状腺良恶性结节鉴别中应用64层螺旋CT的诊断价值。方法选取本院2018年5月至2019年5月收治的86例甲状腺结节患者为研究对象,所有患者均进行64层螺旋CT平扫及增强扫描,记录64层螺旋CT平扫图像特征,以活组织穿刺检查结果为金标准,分析64层螺旋CT检查的准确率、灵敏度、特异度、阳性预测值、阴性预测值。结果恶性结节的边缘模糊率、砂砾样钙化率、形态不规则率、不均匀强化率均高于良性结节,囊性率低于良性结节,差异有统计学意义(P<0.05);良恶性结节的直径比较差异无统计学意义;64层螺旋CT检查的准确率、灵敏度、特异度、阳性预测值、阴性预测值分别为89.53%、87.50%、90.74%、84.85%、92.45%。结论在甲状腺良恶性结节诊断鉴别中应用64层螺旋CT检查具有较高的诊断准确率、灵敏度及特异度,在早期疾病的诊断、临床治疗、预后评估中具有重要意义。
Objective To evaluate the diagnostic value of 64-slice spiral CT in differential diagnosis of benign and malignant thyroid nodules.Methods 86 patients with thyroid nodules treated in our hospital from May 2018 to May 2019 were selected as the study subjects.64-slice spiral CT plain scan and enhanced scan were performed in all patients.The features of 64-slice spiral CT plain scan were recorded.The accuracy,sensitivity,specificity,positive predictive value and negative predictive value of 64-slice spiral CT were analyzed taking the biopsy results as the gold standard.Results The malignant nodules'edge blur rate,without gravel-like calcification rate,morphological irregularity rate,cyst rate,and uneven enhancement rate were all higher than benign nodules,the difference was statistically significant(P<0.05).There was no significant difference in the diameter of benign and malignant nodules.The accuracy,sensitivity,specificity,positive predictive value and negative predictive value of 64-slice spiral CT were 89.53%,87.50%,90.74%,84.85%and 92.45%,respectively.Conclusion 64 slice spiral CT has high diagnostic accuracy,sensitivity and specificity in the diagnosis and differential diagnosis of benign and malignant thyroid nodules.Fracture,clinical treatment and prognosis evaluation are of great significance.
