摘要
目的探讨腹腔镜手术联合促性腺激素释放激素激动剂(Gn Rh-a)药物治疗子宫内膜异位伴不孕的临床效果。方法回顾性分析2014年3月至2015年5月在东莞市人民医院接受治疗的85例子宫内膜异位伴不孕患者的临床资料,根据其治疗方式分为腹腔镜治疗组40例和联合治疗组45例(在腹腔镜手术后继续给予Gn Rh-a药物治疗2个月)。观察两组患者的治疗效果,比较两组患者治疗前后肝肾功能、术后1年复发率、妊娠分娩率和性生活质量的差异。结果联合治疗组治疗有效率为100.00%,高于腹腔镜治疗组的85.00%,差异有显著统计学意义(P<0.01);联合治疗组术后1年复发率为2.22%,低于腹腔镜治疗组的20.00%,1年内妊娠率为88.89%,高于腹腔镜治疗组的70.00%,差异均有统计学意义(P<0.05);两组患者治疗前性生活质量得分比较差异无统计学意义(P>0.05),治疗后,联合治疗组性生活质量得分高于腹腔镜治疗组,差异有显著统计学意义(P<0.01)。结论腹腔镜联合Gn Rh-a药物治疗对子宫内膜异位伴不孕患者有较好的治疗效果,可明显提高患者的妊娠率,且无严重副作用,具有临床应用价值。
Objective To investigate the clinical effect of laparoscopic surgery combined with gonadotropin releasing hormone agonist（Gn Rh-a） in the treatment of endometriosis with infertility. Methods The clinical data of85 patients endometriosis with infertility in our hospital from March 2014 to May 2015 were retrospectively analyzed.According to the treatment methods, the patients were divided into laparoscopic treatment group（40 cases） and combined treatment group（45 cases, which were treated with Gn Rh-a for 2 months after laparoscopic surgery）. The therapeutic effects of the two groups were observed and compared between the two groups before and after the treatment in liver and kidney function, postoperative recurrence rate, the rate of pregnancy and delivery, and quality of sexual life within one year after treatment. Results The effective rate of the combined treatment group was 100.00%, significantly higher than 85.00% in the laparoscopic treatment group（P〈0.01）. The 1-year postoperative recurrence rate was2.22% in the combined treatment group, which was significantly lower than 20.00% in the laparoscopic treatment group,and the rate of pregnancy in 1 year was 88.89%, significantly higher than 70.00% in the laparoscopic treatment group（P〈0.05）. There was no significant difference in quality of sexual life between the two groups before treatment. After treatment, the quality of sexual life score in the combined treatment group was significantly higher than that of the laparoscopic treatment group（P〈0.01）. Conclusion Laparoscopic surgery combined with Gn Rh-a therapy has a better therapeutic effect on endometriosis patients with infertility than laparoscopic treatment only, which can significantly improve the pregnancy rate of patients, with no serious side effects.
