乙肝病毒Enh ∏/BCP/PC基因变异与肝硬化的相关性研究 被引量:4

Associations of hepatitis B virus mutations in Enh ∏/BCP/PC region with liver cirrhosis

摘要 目的探讨乙型肝炎病毒(HBV)Enh ∏/BCP/PC基因变异与肝硬化(LC)的关系。方法采用1∶2匹配病例对照研究方法,通过DNA扩增、测序等方法测定243例HBV阳性肝硬化(LC)和486例非肝硬化HBV感染者血清HBV Enh ∏/BCP/PC基因序列,确定其变异状况。结果 Enh ∏/BCP/PC基因中10个核苷酸位点(nt.1652、nt.1653、nt.1674、nt.1719、nt.1726、nt.1727、nt.1730、nt.1762、nt.1764、nt.1799)的变异频率在HBV B和C型感染者中差异有统计学意义(P<0.05)。单因素分析结果显示,在C型HBV感染者中,G1719T(OR=2.07,95%CI:1.34-3.21)、A1727G(OR=2.63,95%CI:1.57-4.42)、A1762T(OR=3.80,95%CI:2.37-6.11)、G1764A(OR=10.24,95%CI:5.54-18.91)、A1846T(OR=2.25,95%CI:1.28-3.98)、G1896A(OR=2.77,95%CI:1.70-4.53)显著增加LC发生风险,而A1652G(OR=0.51,95%CI:0.27-0.95)显著降低肝硬化发病风险。多因素分析结果显示A1762T/G1764A(OR=2.09,95%CI:1.05-4.17)、G1896A(OR=3.28,95%CI:1.49-7.21)是乙肝肝硬化的独立危险因素。结论 HBV Enh ∏/BCP/PC基因的A1762T/G1764A和G1896A变异是乙肝肝硬化的独立危险因素,对HBV感染者发生肝硬化具有潜在的预测价值。 Objective To evaluate the associations of hepatitis B virus ( HBV) mutations in Enh P/BCR/PC region with liver cirrhosis(LC). Methods A total of 243 patients with LC and 486 HBV-infected subjects without LC were included in this 1: 2 case-control study. DNA amplifying and direct sequencing were used to determine the Enh P / BCR/PC gene sequence of serum HBV DNA and investigate the mutations. Results Frequencies of mutations at 10 nucleotide sites, including nt. 1652, nt. 1653, nt. 1674, nt. 1719, nt. 1726, nt. 1727, nt. 1730, nt. 1762, nt. 1764, and nt. 1799 were significantly different between HBV genotypes B and C( P 〈 0.05). Substitution mutations,including G1719T( OR =2. 07,95% CI: 1. 34 - 3. 2 1 ), A1727G( OR = 2. 63,95% CI: 1. 57 - 4 . 4 2 ), A1762T(OR=3.80,95%CI:2.37-6.11), G1764A(OR = 1 0 .2 4 ,9 5 %CI :5 .5 4 - 1 8 .9 1 ), A 1 8 4 6 T (O R = 2 .2 5 ,9 5% C I :1 .2 8 - 3. 98), G1896A( OR =2. 77,95%CI: 1. 70 - 4. 53) significantly increased the risks of LC in HBV genotype C. Multivariate analysis indicated that A1762T/G1764A ( OR =3. 06,95% CI : 1. 93 - 4. 86 ) and G1896A ( OR =4 . 27,95% CI : 2. 64 - 6. 90) significantly increased the risks of LC. Conclusion A1762T/G1764A and G1896A were independent risk factors for LC,which would be potential predictors for cirrhosis among HBV-infected subjects.
