摘要
目的回顾性分析107例全数字化乳腺X线摄影表现为边界清晰的乳腺肿块患者资料,探讨与肿块良恶性相关的影响因素,以提高诊断准确率。方法搜集术前诊断为BI-RADS 3类和BI-RADS 4A类单发的边界清晰乳腺肿块患者,所有肿块均有手术病理。回顾性分析患者一般资料和乳腺X线摄影表现,包括患者年龄、肿块发生部位、大小、密度、浅分叶、周围晕征、钙化等。运用单因素和多因素分析年龄、肿块最大直径、部位、肿块密度、浅分叶、周围晕征、钙化等因素对肿块良恶性的影响。结果乳腺X线摄影对于边界清晰乳腺肿块在鉴别良恶性方面,存在着征象交叉重叠。分析表明年龄与肿瘤良恶性有关,年龄每增加1岁,恶性危险性随之增加;肿块浅分叶和无周围晕征是肿瘤为恶性的危险因素,有浅分叶的肿块呈恶性可能性是无浅分叶肿块的6.68倍;无周围晕征的肿块呈现恶性的可能性是周围有晕征肿块的5.45倍。有无钙化在良恶性上无统计学差异,但其钙化在表现形态上有差异,恶性钙化细小,良性钙化粗大。结论 X线摄影表现为边界清晰的乳腺肿块,其良恶性与年龄、肿块浅分叶、肿块周围晕征密切相关,综合分析可提高诊断准确率。
Objective To analyze the influencing factors of benign and malignant breast masses in 107 cases with clear margin,and to improve the diagnostic accuracy. Methods All the breast masses diagnosed as BI-RADS 3 or BI-RADS 4,which had circumscribed margins. All of the masses had the pathology results. The general data of the patients was analyzed retrospectively,including age,location,size,density,microlobulation,halo sign,calcification,etc. Single factor and multiple factor analysis were used to analyze the influence of age,tumor size,location,mass density,microlobulation,halo sign,calcification and other factors on the benign and malignant tumors. Results Mammography signs for diagnosing benign and malignant breast masses may overlap. This study shows that malignant cancers are related to age,as the lesion grows each year,the risk of malignancy increases; microlobulations and lack of surrounding halo sign are the risk factor for malignancy. The malignancy possibility for microlobulated masses was 6. 68 times; The malignancy possibility in lesions with no halo sign was 5. 446 times. There was no statistical difference in benign and malignant calcification,but,the shape of calcification had difference. The malignant calcification was small,and the benign calcification was coarse. Conclusion Mammography showed circumscribed masses and malignancy is closely related to the age,the microlobulations,and the halo sign. Comprehensive analysis can improve the accuracy of diagnosis.
