摘要
目的探讨难治性与一般性抑郁患者情绪加工的认知偏见的差异。方法采用视觉情绪偏差的的实验范式(即对高兴和悲伤情绪面孔做出判断)诱发情绪加工过程,对28例难治抑郁症患者(难治组)、32例非难治性抑郁症患者(非难治组)、28名健康被试(对照组)进行事件相关电位N170成分(即在170 ms左右出现的负波成分)测定,测量其N170的波峰波幅。结果与对照组相比,难治组和非难治组高兴和中性面孔信息加工的N170波幅降低(P<0.05),而难治组悲伤面孔加工的N170波幅升高(P<0.05),非难治组悲伤面孔加工的N170波幅差异不显著(P>0.05)。与难治组相比,非难治组高兴面孔加工的N170波幅更高,悲伤面孔加工N170波幅更低(P<0.05)。抑郁患者的悲伤面孔加工的N170波幅与两组汉密尔顿抑郁量表评分分别呈显著正相关(P<0.05)。结论难治性抑郁症患者存在负性情绪信息加工的情绪加强效应的认知偏见。提示难治性抑郁症患者的无意识负性认知偏见可作为临床辅助治疗的一个生物学标志。
Objective To explore the difference of cognitive bias of emotional facial expressions between treatment-resistant and nontreatment-resistant depression patients. Methods Using visual emotional oddball paradigm( i. e. making judgments about happy and sad faces) to manipulate the processing of emotional information,the event-related potential N170 component( i. e. the negative wave component occurs around 170 ms) was tested in 28 cases of treatment-resistant depression( TRD group),32 cases of non-treatment-resistant depression( NTRD group),and 28 healthy subjects( control group). Results(1)Compared with control group,the N170 amplitudes for happy and neutral faces were decreased in both TRD group and NTRD group( P〈0. 05),N170 amplitudes for sad faces were increased in TRD group( P〈0. 05),but no significant difference of N170 amplitudes for sad faces was found in NTRD group( P〉0. 05). Compared with TRD group,the N170 amplitudes for happy faces were increased,while N170 amplitudes for sad faces were decreased in NTRD group( P〈0. 05). A positive relationship was observed between N170 amplitude and the Hamilton depression rating scale( HDRS) score for sad faces identification in two depression groups( P〈0. 05). Conclusion The treatment-resistant depression patients present the mood-brightening effect with cognitive bias on negative emotional processing. It further suggests that the unconscious negative cognitive bias of treatment-resistant depression patients may be a potential biomarker for the clinical adjunctive therapy.
