摘要
目的了解2016年唐山市4所医院医护人员焦虑、抑郁的影响因素及与心理弹性的关系,为采取有效的干预措施提供依据。方法采用随机抽样的方法抽取2016年唐山市4所三甲医院共940名医护人员,运用流调中心用抑郁量表(CES-D)、焦虑自评量表(SAS)、心理弹性量表(CD-RISC)、自编一般情况量表进行调查。结果 (1)医生焦虑及抑郁症状率分别为40.7%和52.3%,护士焦虑及抑郁症状率分别为52.5%和66.0%。护士的焦虑及抑郁症状率显著高于医生(χ~2=17.873,P<0.01;χ~2=12.834,P<0.01)。(2)单因素分析显示,医生焦虑影响因素为年龄及睡眠障碍(χ~2=12.377、χ~2=25.141,P<0.01),抑郁影响因素为睡眠障碍(χ~2=6.619,P<0.05);护士焦虑影响因素为健康状况、近期有无重大疾病(χ~2=20.073、χ~2=5.986,P<0.05),抑郁的影响因素为年龄及近期有无重大疾病(χ~2=10.523、χ~2=9.508,P<0.01)。回归分析显示,焦虑抑郁的影响因素,医生为婚姻状况、睡眠障碍及心理弹性(P<0.05),护士为健康状况、心理弹性及近期有无重大疾病(P<0.05)。(3)医生是否焦虑及抑郁的心理弹性得分比较(t=-10.145、t=-3.353,P<0.01),护士是否焦虑及抑郁的心理弹性得分比较(t=-10.271、t=-2.543,P<0.05);医生焦虑及抑郁(r=-0.446、r=-0.170)和护士焦虑(r=-0.410)与心理弹性呈显著相关(P<0.01)。(4)心理弹性对焦虑及抑郁单独做出的解释,医生分别增加14.53%及1.14%,护士焦虑增加15.69%。结论唐山市医护人员焦虑及抑郁受多方面因素影响,其焦虑抑郁与心理弹性密切相关。
[Objective] To study the influencing factors of anxiety and depression among medical staff from four hospitals of Tangshan City in 2016,explore the relationship between them and psychiatric resilience,and provide the evidence for taking effective intervention.[Methods] 940 medical workers were collected from 4 grade-A tertiary hospitals of Tangshan City in 2016 by random sampling method,and investigated by the Center for Epidemiologic Studies Depression Scale (CES-D),Self-Rating Anxiety Scale (SAS), Connor-Davidson Resilience Scale(CD-RISC) and self-designed general questionnaire.[ Results ] @The rate of anxiety and depression in doctors was 40.7% and 52.3% respectively,and rate in nurses was 52.5% and 66.0% respectively. The rates of anxiety and depression in nurses were significant higher than those of doctors (X^2=17.873,P〈0.01,X2=12.834,P〈0.O1 ). ②The single factor analysis showed that the influencing factors of anxiety in doctors was age and sleep disorder(X^2=2.377, X2=25.1dl,P〈0.01 ),and the influencing factor of depression was sleep disorder (X^2=6.619,P〈O.O1). The influencing factors of anxiety in nurses were health status and having serious disease recently (X^2=20.073, X^2=5.986,P〈0.05), and the influencing fac- tors of depression were age and having serious disease recently (X^2=lO.523,X^2=9.50$,P〈0.01). Regression analysis showed that the influencing factors of anxiety and depression for doctors were marital status,sleep disorders and psychiatric resilience(P〈0.05), and those for nurses were health status,psychiatric resilience and having serious disease recently (P〈0.05). (~There were statis- tically significant differences in score of psychiatric resilience between doctors with and without anxiety and depression(t=-10.145, t=-3.353,P〈0.01 ),and the differences in score of psychiatric resilience were statistically significant between nurses with and without anxiety and depression (t=-10.271,t=-2.543,P〈O.05).
