摘要
目的观察补肺纳肾法治疗慢性阻塞性肺疾病稳定期的临床疗效。方法将60例慢性阻塞性肺疾病稳定期肺肾两虚证患者,随机分为对照组和治疗组,每组30例。对照组采用西医常规治疗;治疗组在对照组治疗方法的基础上,加用中药补肺纳肾,两组治疗及观察时间均为60天。观察并对比治疗前后中医证候积分、FEV_1%pred(FEV_1占预计值的百分比)、FEV_1/FVC%(FEV_1占用力肺活量的百分比)、6MWT(6分钟步行试验)、CAT量表评分的变化。结果两组治疗前后及治疗后组间比较,证候积分减少、6MWT延长、CAT积分减少,并且治疗组表现出更好的疗效,差异有统计学意义(P<0.05);治疗组治疗后FEV_1%pred及FEV_1/FVC%均值较治疗前略有上升,对照组稍有下降,差异无统计学意义(P>0.05)。结论补肺纳肾法能在症状、运动耐力、生活质量上全面改善慢阻肺患者的病情,还可能延缓气道阻塞的进展速度抑制肺功能的下降。在慢阻肺的治疗中,应重视肺肾两脏的功能协调,肺肾同治,可取得更好的临床效果。
Objective： To observe the clinical effect of Invigorating Lung and Nourishing Kidney Method on the treatment of chronic obstructive pulmonary disease （ COPD）. Metliods ： 60 patients were randomly divided into a con-trol group and a treatment group, 30 cases per group. The control group was treated withi cine while the treatment group was added Chinese medicine for invigorating lung sides the conventional western medicine of the control group. The treatment and were 60 days. The changes of TCM syndrome scores, FEV1L pred, F E V 1 /FV C L, 6MWT, CAT scale were observed and compared before and after treatment. Results ： Compared with the two groups before and syndrome score decreased,6MWT prolonged and CAT score decreased. The treatment group showed better curativeeffect,and the difference was statistically significant （ D 〈 0. 05 ） . The mean values of FEV1L pred and FEV1/FVCL of the treatment group after the treatment slightly higher than those of prior - treatment was slightly lower. There was no significant difference between the two groups （ D 〉 0. 05 ）. Conclusion ： InvigoratingLung and Nourishing Kidney Method can improve the condition of COPDpatients in symptom, exercise endurance and quality of life and may delay the decline of airway obstruction. In the treatment of chronic obstructive pulmonary dis-ease , more attention should be paid to the function coordination of lung and kidney and the common treatment of lungand kidney for better clinical results.
